Tanuar ILETZ NEWS A monthly publication by the Confederated Tribes of Siletz Vol. 29, No. 1, January 2001 Contract Health Services (CHS) Update 2001 Pharmacy: The limit for this year is $400 when purchasing at retail providers (Fred Meyer, Wal-Mart, Rite Aid, etc.). All maintenance or long-term prescriptions must be mailed to Pequot Pharmacy. Any questions about mailing should be directed to CHS or your local CHA. Any problems or other concerns about delivery should be directed to Pequot. The telephone number is on the back of your card (1-888-779-6638). Forty-mile radius is a policy that is currently in effect. If you live within a 40-mile radius of an IHS (Indian Health Service) or tribal clinic, you are restricted to using that clinic for medical, dental, pharmacy, and eye care needs. Specialty care referrals must be sent to CHS by Tuesday afternoons for the Gatekeeping Committee meetings on Wednesday mornings. They may be mailed to PO Box 549, Siletz, OR 97380 or faxed to 541-444-9645. Please call CHS directly with any questions. Dental services are now available for adults (please call in your appointments). Services are open for exams and X-rays with treatment plans. All treatment plans must be mailed or faxed in for approval. Deferred services have been reported to Portland Indian Health Service as unmet needs. The level of priority will remain the same. Surgeries and other referrals that cannot wait will be approved after 2001. When they are completed and the restriction on dental is lifted for adults, the Gatekeeping Committee will do a cost analysis to see where we’re at with funds before the level of priority is changed. Please remember to call in all appointments. Pre-authorization is required. If you have an emergency, you have 72 hours to report your visit to CHS.
