year is $400 when purchasing at
retail providers (Fred Meyer,
Wal-Mart, Rite Aid, etc.). All

(1-888-779-6638).
Forty-mile radius is a policy
that is currently in effect. If you live
within a 40-mile radius of an IHS
(Indian Health Service) or tribal
clinic, you are restricted to using
that clinic for medical, dental,
pharmacy, and eye care needs.
Specialty care referrals must be sent
to CHS by Tuesday afternoons for
the Gatekeeping Committee

They may be mailed to PO Box

be done at the IHS and tribal clinics.
Crowns, dentures, and root canals

approved in private practice.
Please remember to call in all

Preferred providers are
currently contracted. Please call if
you have questions on referrals
made to providers you are not
familiar with. Corvallis and
Philomath residents should use
Samaritan Health Care providers.
Albany, Lebanon, and surrounding
areas are restricted to Chemawa or
Siletz clinics. Corvallis Clinic
should be used for specialists only.
If you have questions, call CHS
directly at 1-800-628-5720 or
541-444-1236.
Deferred services have been
reported to Portland Indian Health
Service as unmet needs. The level
of priority will remain the same.
Surgeries and other referrals that
cannot wait will be approved after
2001. When they are completed and
the restriction on dental is lifted for
adults, the Gatekeeping Committee
will do a cost analysis to see where
we’re at with funds before the level
of priority is changed.
Please remember to call in all
appointments. Pre-authorization is
required. If you have an emergency,
you have 72 hours to report your
visit to CHS.
See candidates’ statements on pages
7-12 and budgets on pages 22-23.
