July 26, 1956 The Medical Roundup A Painful Tailbono, or Coccyx Quite a fow highly nervous and over-sensitive women com plain of a sore tailbone, or coccyx which, in us humans, is a remnant of the tail in animals. It contains s e v eral little bones fused to gether. By put ting bis index finger in the Hallrt Alvarn lEivummiu uic thumb on the back of the tail bone between the buttocks. a physician can easily examine the joints between the little tail and the sacrum (the bone which is found between the two hip i bones). He can move the little tail up and down, and he can see if there is any roughness or sign of inflammation about it. 'He can also have x-ray studies made of the little bone and its ! joint with the sacrum. Hundreds of times I've studied the coccyx in cases in which the woman uas complaining of con stant pain, and never have I found anythins wrong with the bone or the joint. Usually, mov ing the coccyx up and down did not produce the pain complained of. I think it highly significant that these distresses and pains around the coccyx are found al most always in women and only rarely in men. Women complain more often of nervous troubles, while men complain more often of serious demonstrable disease. The women who complain of a sore coccyx are almost always a bunch of nerves, and they often have relatives who are also high ly nervous or a bit psychotic. I wish I could tell what to do to make their coccyx more com fortable, but the best advice I can give them is not to waste their money, and certainly not to have an operation. Years ago. when I was a young man. surgeons used to remove the coccyx to try to make these women happy, but I never saw the operation do any good. The surgeons must have had the same bad experience as I did because alnmst ail of them quit doinu this work. Pain in the Rectum Many women of a nervous type complain bitterly of con stant pain around tiie rectum. But. again, an expert can exam ine the rectum very thoroughly and he can even open the pa tient's abdomen and look in. and he will practically never see any cause for the distress. An impor tant point is that these pains around the coccyx and in the re gion of the rectum are not made worse by constipation, by mov ing the bowels, by diarrhea or by anything. The distress is practic ally constant, and this is typical of all pains that are arising up in a disturbed brain. Pains that arise in the brain are often re ferred out into some perfectly normal part of the body some where. The situation is much like that in the amputated leg of an old soldier, who keeps saying that his toes feel terribly uncom fortable. This is what is called phantom pain. Diverliculosis Irr spite of everything I keep writing about diverticulosis and diverticulitis of the colon I keep getting anxious requests for a diet and treatment for diverticu losis. For long. I have kept say ing that, so far as I can see, di verticulosis of the colon is a symptomless condition that com es with age. It is like gray hair, and I would just as soon give a diet for gray hair as for diverti- ! culosis. One great trouble is that j hundreds of laymen and, I fear, j some of us physicians, get mixed ! up as to diverticulosis and diver ! ticulitis. Dozens of persons who write, saying that they are in a bad way with diverticulitis have ' not a symptom of this serious, acute, painful, feverish and dan gerous disease. All they tell me is that a lot of diverticula, or tiny out-pouchings, were discov j ered accidentally on an x-ray film. Diverticulitis means n 't flammation of some of the ticula. Diverticulosis is ju: I having of a few pouches. . . months ago, when I went foi . I routine yearly x-ray study, they found thre diverticula on my col on pouches which were not there last year when I was ex- i arr.ined. j Am I excited about them? No. Am 1 going to do anything about them? No. Do they constitute any danger to my life, or health, or happiness? I very much doubt it. I once went for some five years at the Mayo Clinic without see ing a single case of diverticulitis, which showed me how rare the condition is. Every year I see a number of people who were told they had diverticulitis, but ac tually someone got mixed-up with his words, and all the per son had was some harmless di verticulosis. Some of my physi cian friends may ask. "How do you know they are harmless?" And my answer is that I have known hundreds of healthy per sons whose colon was full of them. Dr. Alvarez hopes his readers will understand that it would be impossible for him to answer re quests for information or to at tempt to diagnose by mail. ' In order to get control an in secticide must be applied to the silks some time after they emerge but within three days. One application of insecticide will protect the ear during its growing period, but as all ears do not produce silks at the same time, it is necessary to make two applications to the patch.
