Moore, lato BurReon-Qoneral with tho Government of Bombay, has a paper in the Hospital on "Tho Immediate Treatment of Accident and Sudden Illness," in which ho statos that only iho other day ho was passing Kensington Gardens, when he ob served an old man being supported by two other persons, and encouraged to cling to tho railings, so as to maintain an upright posture. Aglanco showed what was the matter with tho old man. Ho had been struck by cardiao failure, or serious apo plexy, or that variety of tho malady in which tho faco is pale, and tho circulation of tho blood f ailing. His pulso was feeble and intermittent, anrt thero was ovident paralysis on ono sido of tho body. Now, what ought to have been done in such a case? Tho poor old man should havo been laid on right sido full length, his collar should havo boen opened, ho should have been allowed plenty of fresh air, and his legs, feet and hands should havo been well ruDDea. men, when tho first shock of tho malady had passed away, ho ought to havo boon carried, still in tho recumbent posture, to tho nearest hospital Again, another in stance. Only a fow days back, in tho Ham mersmith road, two boys were knocked down by a cab. Ono escaped with little in jury, tho other got concussion of the brain. Ho wa3 almost senseless, speechless, cold and pallid. Tho samo plan ought to havo been pursued as in tho case of tho old man; but instead of this, tho boy was sur rounded by a dense crowd of innnfattivn people, and one good but mistaken Samari tan was supporting tho injured youth in an upright position on his knee. After a-tlmo the poor boy was dragged away on his legs, being well shaken and jostled in tho process. Yet a third instance. In Hydo Park, ono recent Sunday, a person was taken in an epileptic fit. Ono bystander wisely ran for water; others carried tho epileptic to a bench, on which he was seated. Now, in epilepsy thero is a great struggling, the limbs are convulsed, and tho tongue is very liable to bo protruded between tho teeth and severely bitten. This accident should bo guarded against by placing some thing, as a small pieco of soft wood, too largo to slip into tho mouth, between tho teeth; atldtho person should bo simply pro vented injuring himself in his struggles un til the fit passes away. But nothing of tho kind was attempted; tho consequence being that the tongue was bitten, and tho limbs of the epileptic were bruised-against tho hard wood of tho bench on which ho was seated and held. Had ho boen placed on the soft grass and left alono ho would havo injured himself less. Tho following aro rules which may bo safely adopted in cases ol accident and sud den illness. Placo tho porson on tho ground or floor lying toward tho right side, andwith the head raised to the lnvii nt , HIN body by n pillow, folded coat or other soft substance. Then unbutton or split open any clothing pressing upon tho neck or chest. Tho faco and chest should bo sprinkled with cold water, and some cold water may bo given to drink if the power of swallowing remains. Wino, brandy or other liquor should not bo hastily given without evidence of their being needed. Ex amine 'the head and limbs separately. Tho prominent parts of tho limbs may bo examined with very Uttlo movement, of tho body. If it is necessary to move a person after any injury, especially of tho head, tho person should bo earned while Jying down. Ho should not bo allowed to sit upright in a vehicle or to walk. Seo that tho person is allowed plenty of fresh air. The history of tho accident should be obtained, and when sending for a surgeon the messago should bo as clear as possible, and, if prac ticable, a written ono. The most common maladies which occur suddenly to people in tho streets of largo towns are apoplectic and epileptic at tacks, maladies depending on heart affec tions, and drunken fits, although, perhaps, the latter should not bo classed under the head maladies. But it is necessary to dis tinguish apoplexy and epilepsy from "dead drunk," as it is called, and also poisoning by opium. Apoplexy is known from epi lepsy by tho puffing or snoring breathing which occurs in tho former malady. In epi lepsy thero is no struggling of tho limbs, the eyes aro turned up under tho lids, so that tho Whites only aro visible, and tho person generally falls down with aloud cry, none of which are tho symptoms of apo plexy. Apoplexy is to be distinguished from affections due to tho heart, because in the latter maladies the symptoms are very similar to fainting. The person is pale, un conscious, with feeblo pulse, scarcely per ceptible breathing, white lips and a death like expression of countenance; whilo in apoplexy, though tho person is unconscious, there is snoring or puffing breathing, and the face is often drawn to ono sido. Apo plexy is best distinguished from the effects of spirituous liquors by the history of the case, which may probably be one of drink ing. Secondly, by tho smell of liquor in tho person's breath; although this is not a cer tain sign, for some one may, in mistaken kindness, have given tho person struck with apoplexy some kind of liquor. Thirdly, in the " drunken fit," tho size of tho pupils of tho eyes is equal, whilo in apo plexy ono is often larger than tho other. By the pupils of the eyes is meant the round space in the center of the dark part of tho eyo. Fourthly, the person "dead drunk" may generally bo roused, when 'ho babbles 'incoherently, whilo from apoplexy the person can not be roused. Lastly, if any movements occur in drunk enness they will be of the limbs, whereas movement is usually restricted to one side of the body in apoplexy, From poisoning by opium apoplexy is best distinguished by the history of the case ; by tho smell of opium in the breath; by both pupils of the eyes being very small and contracted; by the fact that tho patient may bo roused, al though he does not then babble as in the drunken fit, but immediately goes to sleep again. If all that has been advanced can not be held in recollection tho condensed rules may certainly be remembered, viz. : First, in all cases of accident or sudden illness let the person lie down, and do not assist him to maintain the upright posturo; secondly, let him have plenty of fresh air; thirdly, unloose all clothing round the neck and chest; fourthly, convey tho patient, still in . the recumoeni position, u mo nearest hospital. Stretchers for this purpose ought to be available at known points, or at least at every police station. -$, Conscience unit KreuUert. Still ho sticks to the old family physician, nnd the physi cian knows and has known from the beginning that the patient has been stricken with death for months, for he knows full well that the profession acknowledge they havo no remedlos for the cure of Kidney Dieoaie.
