The Medical Roundup MEDFORD MAIL TRIBUNE. MEDFORD. OREGON Emeritus Consultant tn Mtaiclna .Mayo cunir Emeritus Protestor of Mediclna Mayo CHitic (Recihter and: Tribune Syndic!, 1963) Tenosynovitis People occasionally ask me about tenosynovitis. One wom an, writes asking me to tell her "all I know about it." ' l&&?ti Actually, I do .-'i"rS n 0 1 know r fyl much. "Teno" ' '. -t&!s relers to i l&-2aSp4 tendon, which J-.:,t corc' running ; - ' f US,? 1 (usually) to a ..-JT 1 hnnp Fnr in. r ' - I i " " msS i s'ance, when A" arM i want to grao omething and hold it firmly In my hand, muscles in the upper part of my forearm contract and pull on tendons that run to the palmar side of the several bones in my fingers. As I now glance at the back of my hand, I can see four tendons running to the backs of the bones in my fingers. When muscles pull on these tendons, they cause my hand to open. Around each of these ten dons is a little sheath, lubri cated with a fluid called "synovia." Hence, tenosyno vitis means an inflammation of the shreath around a ten don. Always, then, there is some soreness or pain, and there may be a . little swelling and some reddening of the skin over the inflamed area. The next questions is, "Why does the inflammation come?" I imagine it comes most often in people like me who for all their days have spells of "fibrositis," an arthritis-like disease which causes some ir ritation or inflammation of the several fibrous tissues in the body. In my case, every o often I will have some soreness around a joint, I may have enough pain in a hip joint or ankle joint so that when I get up in the morn ing, I will have to limp a bit. When that happens, I take a walk and usually after I have gone three blocks, the pain is gone for the day. A spell of pain may last for three days, a few weeks or, rarely, a few months. Occasionally, I will get a mild spell of tenosyno vitis and it may hurt to move my fingers. But usually, in a few days or weeks, the dis tress is gone. I never take any medicinie for it. If it were to produce enough pain to both er me, I would take some aspirin. One of my correspondents wants to know if he should be operated on for his teno synovitis, I do not know why he should be. He could be if he has a rounded soft nodule. called a ganglion, on the back of his wrist. This is usually a little bag of synovia, attach ed to a tendon sheath. It can be cut out of if it is causing too much pain or inconveni ence or deformity. The important point is that tenosynovitis is rarely a seri ous disease that should cause worry. Without any treat ment, a spell is likely to clear up. It is possible that it would go away sooner if the person could have some expert physi otherapy, perhaps with light massage and contrast baths, in which a hand or foot is shift ed from cold water to hot water and back again. The term "arthritis" is used to designate any one of the several types of trouble in joints. Fibrositis is often con fused for the more severe arthritis. Additional informa tion is available in Dr. Al varez's booklet on "Arthritis and Rheumatism." Drowning Victims Taken From Lake Kelso, Wash.-tUPD- Skin-divers Sunday recovered the bodies of two of three drown ing victims from the icy depths of Spirit lake at the foot of Mt. St. Helens 45 miles east of here. The bodies of Aaron Bur ton Casad, 23, a teacher and coach at Castle Rock High school, and Allen Larson, 16, Castle Rock, were recovered after skindivers reported searching in depths of as much as 150 feel. Skindivers and grappling crews continued to search for Larry Grubb, 18, the third victim of a boating mishap on the lake Saturday. Four other Castle Rock high school students, all members of a Sea Scout troop, made it to shore in life-jackels after their 10-foot outboard sank when it collided with blocks of ice on the lake. millions.... Of course, if you never knew about that sim ple but necessary adjunct to a man's wardrobe - the 'boot jack" - you will only see in the seed of the Spanish needle plant a two-pronged, flat seed that waits, along with hundreds of its fellows, to adhere to your clothing or the fur of any passing ani mal. That's the idea, a clever scheme whereby this noxious weed sends its seeds to start new colonies far away from the parent plant. The Spanish needle's idea is well founded, and effective. Back a few generations ago men wore boots, not the rub ber boots today's firemen wear to keep their feet dry, but high topped leather boots. The leather in those old boots were not the soft, pliable va riety we have today, but poor ly tanned, heavy leather that, when it was cold, was about as "stiff as a board." Boots Tailor Made The well dressed gentlemen of yesteryear probably had his boots tailor made, and of a rather close fit - so firmly fitted, in fact, it required considerable pushing and stomping to get the feet snug ly seated. This was often ac complished by the man jump ing up and down, acutally driving the boots onto his lower legs and feet. Every thing was just dandy when putting boots on, but when night came and the man was ready to retire, a problem presented itself. If the man lived alone he had to employ bootjack. Otherwise, he couldn't get his boots off, which probably explained why it was honorable to "die with one's boots on:" dying was easier than removing the tight fitting footwear. The bootjack was a simple arrangement: just a board with one end raised off the floor a couple of inches, and two prongs extending from the higher end. The heel of the boot filled in between the two prongs. The man stood on the board, placed the heel of the other boot in the slot and by muscular pull yanked off the boot. He changed feet and pulled off the other. So simple was the idea, it was silly. Whoever the character was that figured the silly thing out in the first place probably thought himself a genius. Boots Protected Legs The high, leather boots the dandy of yesteryear wore also kept the bootjack's counter part, as well as namesake, from cluttering up his lower limbs, for the bootjacks that are the seeds of the Spanish needle plant couldn't send their sharp pronged barbs into leather. It would have had to be a pretty stupid individual that didn't notice the striking sim ilarity between the seeds of the Spanish needle and the old fashioned bootjack; they look exactly alike. Because the two prongs on the seer of the plant are sharp, they were named "Devil's boot jack." The flowers that appear on this common weed are showy, ;ven pretty, and are of a golden yellow color or a pasty white. The leaves of the plant resemble those of the mari gold. The seeds form a small ball, the blunt ends imbed ded in the flower base and the sharp, two pronged ends outward, where the Larbj cv.n hook onto any passing ani mal. Those folks who never saw a bootjack often call the seeds of the Spanish needle "Devil's pitchforks,", especial ly if the seed is equipped with four barbs instead of two. Less imaginative folks just call them "stickseeds."
