In spite of the fact that sev eral times a year I say in this column that I don't believe that the giv ing of the usual tiny doses of fe male hormone to a meno pausal woman will ever pro duce cancer, I get many re quests for in- or. Alvarez formation o n this point. It is sad that so many thousands of us phy sicians still fear the use of female hormone. We got the idea into our heads years ago when someone gave enormous doses of female hormone to tiny mice, and gave it to them for practically the whole length of their short lives. As a result, a few of them de veloped cancer. Obviously, there is a tremendous differ ence between the conditions in this experiment and those present when a menopausal woman who weighs perhaps 130 pounds, is given half a milligram (a tiny amount) a day of slilbestrol for six months or so. 1 I, personally, have treated hundreds of women with stil bestrol or other ovarian hor mones, and so far, I have never seen anything to make me think that the drug has produced cancer. I have talk ed to men who have treated many elderly women with huge doses of female hormone to stop their spine from caving in, and I have talked to men who are doing research on cancer, and they all tell me that the chances of our pro ducing the disease in women by giving tiny doses of female hormone are practically non existent. The, Person With Two Dozen Questions ' Often I feel unhappy about it, but I just cannot attempt to answer a six-or seven-page letter containing perhaps a dozen questions. Sometimes, in order to answer these ques tions, I'd have to go to the library and spend a day read ing. For instance, here comes a letter from a woman who, after giving birth to a child, got some phlebitis in one of her thigh veins, such as is ordinarily called a "milk leg." Now she wants to know; (1) what caused the painful spot to come in her thigh? (2) is it phlebitis? (3) can It be removed with an operation? (4) should it be removed by an operation? (5) will it spread to other parts of her body? (6) how soon will it start "making tumors" all over her body? (7) if an operation is performed, will it be success ful? (8) will she get cancer from this thing? (9) how long should she have remained in bed after the vein became in flamed? (10) Will walking be bad for it? (11) if she has another child, will she be likely to get a flare-up of the phlebitis? (12) if she has another spell will it be worse than this one? (13) was the doctor's treat ment of the disease correct? (14) if not, what should he have done? (15) should she be wearing an elastic bandage? (16) if she should be wearing an elastic bandage how long should she keep on wearing it? (17) would it be wise to raise the foot of her bed at night? (18) would x-ray treat ment help? (19) when her daughter has a baby, is she likely to inherit this tendency? (20) if so, is there any way of preventing it? (21) my cor respondent's doctor suggests her taking a drug which will keep her blood from clotting; should she do this? (22). how large' then should the dose be? (23) is the drug likely to do any harm? (24) will an electric pad be likely to help dissolve the clot in the vein? (25) would it help, as the doctor says, for her to take off some of her excess weight? (26) does this trouble have any relation to the fact that her mother had a milk-leg with her first baby? And while the woman was asking questions, she went on and told me about the curious little tumor her son has on the lower end of his spine. (1) Should it be removed? (2) would that be the best treat ment? (3) would it be safe? (4) if left untouched, would it turn into a cancer? These are all good ques tions, and I think the woman should have answers to them; but I'm very sorry, when 50 letters like that arrive in one day, how can I possibly read all of them, let alone answer them? I imagine my corres pondent tried to get answers to these 30 questions out of her doctor, but he, poor fel low, with perhaps another six calls to make before he had to hurry to his office, or try to get home for a late dinner, just grabbed his hat and ran! Conceive? Many women write to ask if, while they are nursing their baby and perhaps not menstruating, are they per fectly safe so far as conceiv ing is concerned. I put this question to Professor Jack Greenhill, one of America's great teachers of obstetrics, and he said that woman is safe for only the first two months. After that, she can not be sure.
