8A MAIL TRIBUNE, MedforrJ, Oregon, Thursday, April 6, 1959 Wilson School to Hear Dr. Young Wilson school PTA members will have a chance to find out about "Unusual Ways to Stim ulate Reading by Children" Friday at 7:30 p.m. Kenneth G. Young, director of curriculum for Siskiyou county, California, schools, will present an hour-land talk on ways to help children read. Mrs. Audrey Nelson, pro gram chairman, said Dr. Young's talk will cover such areas of reading as local and state historical material on livestock brands, local history and government, and local lit erature and music. Portions of his talk will be illustrated with slides and tape record ings. Mrs. Nelson hopes that all parents will take advantage of hearing the dicussion, partic ularly during National Li brary Week. Libarary Week, with the theme "Wake Up and Read," ends Saturday. For years we doctors have estimated the activity of the thyroid gland with a machine which measures the amount of oxygen used by the patient in an interval of time. Usual ly, I don't bother with this measure ment because so often I can tell by look ing at the pa tient and feeling the tempera ture of her hand whether her thyroid gland is too active or too sluggish. If .it is too ac tive, when I shake her hand, I may notice that it is fever ish; she is likely to be fidgety; she may be perspiring exces sively; she may be blinking her eyes; she may have a tremor in her hands; she may think that the room is too warm; her eyes my be promi nent; the skin of her face may have a ruddy color; her pulse may be over 110 beats a min ute; she may be having much insomnia; she may be irrita ble and at times unreasonable; and she may have a little goi ter. If, however, the woman sits quietly in my office, if her skin is cool, if she isn't per spiring, if she hasn't lost weight, if she sleeps well and doesn't feel too warm; and if her pulse is normal, I can't believe . that she has much hyperthyroidism. If she has a goiter, I know it is a fairly harmless rubbery or colloid typ. If it is growing large enough to press on her wind pipe ,or to look ugly, I will ad vise that it be quickly re moved by a surgeon. Slow in Movements Another type of woman may be stout, with a pudginess about her eyes. She may drowse as she waits in my re ception room. When she comes into my office, I may note that she is a bit slow in her move ments and her talk. She may tell me that she feels as if she were thinking through a veil; she feels slowed up and stupid; her skin and her hair are dry; she does not perspire much, and she may feel much too cold; she may have lost what little sexual interest she had in her husband. She may complain of feeling very tired and weak. Her husband may say that her voice has gotten a bit husky, and'she can no longer sing. I hardly need a machine to tell me that this woman probably has al most ho thyroid function left. I can prove that my diagnosis is correct by giving her sev eral grains of thyroid extract a day and then watching her brighten up. Today, the fashion is chang ing, and some doctors are say ing that the old breathing machine often gives inaccur ate results. Now we are sup posed to use only the new test of so-called protein-bound-iodine in the bloocL But this is not working out perfectly. The test is too ticklish for the av erage laboratory worker to use. The worker pught to be an expect chemist. Why? Be cause he or she is going to try ,to measure a few million ths of a gram! That would be bad enough but, in many a case, some drug that the pa tient once took can complete ly "ball-up" the result. Enormous Result For instance, this last week I saw a patient with a small, harmless goiter who had been told that she must have it out immediately, because she had a tremendous hyperthyroid ism. Why did the doctor think this? Because one measure ment of her protein-bound-iodine, or PBI, gave an enor mous result. On watching her for a few minutes and talking to her, I could not see that she had a single sign or symptom of hyperthyroidism. She seem ed to me a perfectly normal woman with a slight goiter. Why, then, did she have the remarkably large amount of protein - bound - iodine in her blood? Because, a month be fore, she had had her gall bladder x-rayed, and apparent ly much of the medicine she had taken for that test was still in her blood! What is extremely unfortun ate is that we physicians and you laymen are each day com ing more and more to worship laboratory reports as if they were gospel truth. We do not seem ever to think that a lab oratory girl or an x-ray man could sometimes make a mis take. We forget they are hu man and not god-like." Also, we forget tht copyists can make mistakes. A fine, healthy-looking business man came into my office to say that he had a hopeless scattered cancer that was supposed to kill him in a couple of months. He wanted to know if there was any remedy I could suggest. I said, "But wait a minute; you don't look the least bit ill. Have you any symptoms of illness?" "No," said the man, "I feel fine. I went to the doctor just for an annual check-up, and he found a red blood sedimen tation rate of 115 mm. in an hour. He said that such a high rate always means a soon-to-be-fatal cancer." Secretary's Error I immediately sent him for another test (his physician ob viously should have done this) and the report came back 3 mm. in an hour perfectly normal. I then telephoned his home physician to ask him to look up to see how the mistake was made. In a few minutes he called back to tell me that .what had happened was that a secretary had copied the re port for the blood sugar in the space reserved for the sedi mentation rate!
