He would have called the woman normal because her basal me tabolic rate (the amount of oxygen used per-minute) was only io or 12 per cent less than the estimated normal for the person's sex, weight and age. At the Mayo Clinic we never thought of treating a person for hypothyrodism un less the symptoms were typi cally those of the disease, and the basal metabolic rate was around minus 20 per cent. Actually, we had a few peo ple on the Staff who were wide-awake and who felt well in spite of the fact that their basal metabolic rate was 25 per cent lower than the esti mated normal. To have treat ed them for a disease would have been like pouring medi cine into one of my very heal thy friends simply because he has the misfortune to be only four feet ten inches tall. I used to see each year a score or two of patients who had been told that their basal metabolic rate was minus 40 per cent. I was sure this was wrong because, when these people came in, they show ed absolutely no sign of hypo thyroidism; most of them were thin and wide-awake, and were working hard in an office. Then Dr. Bradley C. Brownson, examined the in strument with which a number of these measure ments had been made, and found that it was badly out of order. The young woman who had used the instrument had not kept it in good working condition. User Can See At Glance I was delighted the other day to see an instrument for the measurement of basal metabolic rate so designed that the user can see at a glance whether the soda lime that is needed to absorb the carbon dioxide in the Ppatient's exhaled air is fresh and granulated and suitable to do its work. As a result of this change in the construc tion of the instrument, I am no longer seeing wide-awake and obviously healthy people with a report of a basal rate of minus 40 per cent. A person with a minus-40 per cent metabolism should havea pudgy face with per haps thickened eyelids. The woman (it is usually a wo man) might go to sleep in my receptiorr-room; her answers to my questions might be slow, and she would probably feel cold, slowed-up, and un able to think clearly. Com monly, I make the diagnosis of severe hypothyridism as I pass a woman on the street. Many times a year I see a woman who says that some one told her that she had hypothyroidism and put her on two or three pills a day of thyroid substance. With this, she promptly got jittery, nervous and upset, which showed that the laboratory report was wrong, and she did not need extra thyroid substance. If she had needed it it would have made her feel like a new woman. Much-Needed Sermon I might use what I have just written as a text for a much-needed sermon on one of the great curses of medi cine today. Mayo Foundation Emeritus Professor of Medicine, Mayo Foundation men, that the only way in which to diagnose disease is to send the patient to a labo ratory. There is no question that in many cases laboratory and x-ray experts can be wonderfully helpful in the making of a diagnosis, but there is no question also that in hundreds of thousands of cases the diagnosis cannot possibly be made by any of the c o m m o n 1 y-used labo ratory and x-ray studies. The difficulty is that the person's symptoms are all arising in the nervous system. For in stance, there is no ordinary laboratory test that will tell me that the sad-looking wo man sitting on the other side of my desk is going insane, or has a mild, unrecognized form of epilepsy, or is dy ing of a broken heart. All my teaching life I have been begging my graduate students to take better histo ries and to listen with care, and for at least a half-hour, to the patient's story. In cases in which the diagnosis is very difficult, I often keep listen ing to the patient's story for days, until suddenly perhaps, the cat will come out of the bag, and t will know what is wrong. For instance, once I quizzed a very nervous wo man for at least half an hour each day for 10 days, until it came out that always before she got a spell of her severe abdominal pain, - she got a throbbing headache over one eye, like she used to have when she was a girl, getting sick headaches. Obviously, she had a migraine, and when I stopped each attack quickly with an injection of Gynergen, she began rapidly to gain back her old health and weight. Do Men Have A Change Of Life? Many people ask for in formation about the supposed menopause in men, and all I can say is that I do not be lieve there is such a thing. Men of 50 or so sometimes go through a period of mental turmoil, but I am very doubt ful if this is due to any change in their glands of in ternal secretion. Certainly, I have seen men of 50 go through a nervous storm, but the fact that, at 70, the man was still keen and wide awake, and physically and sexually young showed me that his sex glands had -not shriveled - up and lost their function as a woman's ovaries do when she is 50. Another fact that makes me doubt that men of 50 have a change of life is that they seldom get any help from taking male sex hormone.
