' Emeritus Consultant In Medicine, Mayo Clinic Emeritus Professor of Medicine, Mayo Foundation An Enlarged Lymph Gland People ask what causes an en largement of one or more lymph nodes, usually felt in the sides of the neck. If I could see the person, I would note if the enl a r g e ment of the nodes is with in limits of what one sees in many sensi t i v e persons Alvarez whenever they get a fever. If the nodes were a bit larger than that, and if the person had been having a fever, I would have blood tests made to see if he had an infecti ous mononucleosis, a so-called glandular fever which, after a few months, should clear up. If the nodes are decidedly large and the person is young or middle-aged, and if an x-ray examination of the chest shows masses, and perhaps I find more large nodes in the armpits or groins or the abdomen, and if I find a high red blood sedimen tation rate such as goes with a cancer-like disease, I will diag nose Hodgkins lymphoma. I will probably have a surgeon remove one of the nodes through a small incision, and will have a pathologist section it and study it under a microscope. Then I can be sure of the diagnosis, and can go ahead with x-ray treatment. ' In the case of an older person I may note that the big node in the neck is hard, and stuck to surrounding tissues, as in the case of cancer. Then, with the x-rays I may find the original growth in the chest, the stomach, the breast, or the Inner lining of the skull, or. in the mouth or back of the tongue or the thy roid gland. If there should still be doubt as to the nature of the nodule I will have it removed for study, and he. may tell me that, from the microscopic structure of the cancer, it came from the stom ach or lung or other organ. In writing all this I hope I do not scare many young people out of their wits. They should remember that a moderate en largement of the nodes in the neck comes with many colds and fevers. A large node in the groin may be due to some ulceration of the leg or foot. Head and Hand Tremors Many ask what to do for a head tremor, and I have to say I don't know of anything. Some thing must have happened in' the brain which interferes with that steady control of the mus cles which most of us have. Sometimes I suspect that the trouble is due to little strokes, and occasionally I learn that it runs in the family. Relatives have had it. There are a num ber of coarse tremors' of the hands, which are definitely hereditary. A person may say. My father and my grandfather didn't have it," but if one can look up the father or the grand father and test him, one may find that he has the hand tremor in a mild form. It comes out only under certain circumstances. Electrical Burns Problem in Surgery Chicago OP) A plastic sur geon reports that electrical burns are an increasing problem in surgery. Dr. G. Kenneth Lewis told the American Society for Surgery of the Hand that. 965 persons died in 1954 from electrical shock. Lewis, clinical associate at the University of Illinois medical college, said the extent of elec trical burn damage sometimes cannot be determined for days o.- weeks after the accident. He said anything above 65 volts may be considered danger ous, especially if the victim is well grounded. The conditions of contact are more significant than the nature of the current, he said, and moisture increases the danger. Current which may kill one person may have little or no ef fect on others, Lewis said. Elderlj persons, those suffer ing from hardening of the arter ies, alcoholics. Individuals with disturbed thyroid and those with chronically greasy skin are more susceptible to shock, he said. He said a person with heart disease can sometimes withstand as much shock as one with a nor mal heart. HARD WORK RECOMMENDED Springfield, Mass.
