He des cribes the Kai ser Health Plan, which in California and Was hington has 1 0 hos pitals a n d 25 out patient clinics, staffed by 500 doctors. The service costs $4.30 a month for a single mem ber; $7.60 for a subscriber and one dependent, and $9.50 for a subscriber and two or more fam ily dependents. For his money a subscriber receives the serv ices of general physicians and specialists in his home, in a doc tor's office or in a hospital, in cluding laboratory work and x ray studies. An extra charge of a dollar is made for each office visit and $2 are charged for the first home call in connection with each illness or injury. There is no charge for subsequent home visits. Full hospital care, including a complete nursing service, up to 111 days is provided for each illness or injury in the course of a year. There is no fee for an operation other than that of the removal of tonsils and adenoids, for which there is a charge of $15. No charge is made for am bulance service within 30-mile radius of a Kaiser hospital. All medicines are provided during hospitalization. Full maternity care for the subscriber costs $6 after membership has been in force for 10 months. Mental disease is excluded from coverage, but in Oakland and San Francisco the Kaiser Group maintains psychiatric clinics in which a charge of $5 a visit is made. Additional Days Half Rate F a m i 1 j dependents of sub scribers get 60 days of full hos pital care a year for each illness or injury, with 51 additional days at half the private rate. As in any good clinic, the Kai ser doctors work under one roof, sharing facilities, income and patients, and pooling their tal ents. So far as possible, sub scribers to the Kaiser Plan select a particular doctor, but, accord ing to Deutsch, they cannot al ways see him when they go to the clinic. .. Deutsch said that most of the Kaiser doctors - who were in terviewed seemed pleased with their work. They are given a vacation each year and time off for attending medical meetings and graduate courses. They en joy working without having to bother with all the economic problems of running an office and collecting fees. After the only complaint of the doctors was that their work-load of pat ients was too heavy, and that the appointment schedule was often so tight that they could not stop to- give a patient all the time they would like to have given. The main unhappiness of these doctors has come from their hav ing been ostracized by their col leagues in the state. However, with time, there has been a slack ening of the attacks by organized doctors, and now the medical societies are sponsoring prepaid health plans of their own, de vised to meet the Kaiser com petition. Unfortunately, we doc tors have always been very slow to accept changes in the mode of practice of medicine, and most of us have fought advances without suggesting any practical solution for the economic pres sures that have come with mod ern civilization. We are still frowning on group practice al though we know it is best for patients. In the old days a workman with a smashed thigh never got back to work, and so his doctor had to take care of him for no thing. As Deutsch says, group prac tice is the most economical, if only because the facilities in lab oratories and x-ray departments are so fully used that the cost of the service is widely spread. When a physician who sees only a, few new patients a day main tains his own laboratory and x-ray department the cost to him per patient is prohibitive. Every one who ever, in a big city, has gone from one consultant to an other, waiting each time for an appointment, waiting for the doc tor, and then paying a good sized bill, knows what an ordeal it is to get a thorough examina tion made outside of a big clinic. Uufortunately, most of we physi cians are so inclined to be a prima donna that we cannot work in a group. Can Make Friends . Most of we physicians have feared that a good friendly physician-patient relationship cannot be maintained in a big clinic, but I, who for 25 years worked in a group, know that in it one can make dear friends of hund reds of patients. Fortunately, the opposition of solo physicians to group practice is dying out. Big clinics are here to stay; more and more of them are going to be . built, and we physicians who work alone will have to face this fact. We must also face the fact that more pre paid clinics of the Kaiser type will be built, and will flourish because thousands of persons are glad to get the services they give. Dr. Alvarez hopes his readers will understand that it would be impossible to answer requests for information or to attempt to di agnose by ma'il. (Released by The Register and Tribune Syndicate, 1957) Tree Planting Set By Forest Service Cave Junction A 12,000 tree planting operation in the Long Gulch Lakes area is scheduled to start this week. The two-year-old Douglas fir trees planted by the forest service will cover approx imately 40 acres. According to forest service of ficials, a crew of four men has completed planting 10,000 two-year-old Ponderosa pine seed lings in the Hogue woodlot tim ber sale area on the East Fork of the Illinois river. About 65 acres of the Crazy Peak area and about 61 acres near Althouse creek will be re seeded by helicopter after the contract is let April 10.
