Mayo Foundation Diverticulosis and Diverticulitis Many readers of this column have begged me to send them reprints of columns I have writ- l"'Jf-ii!ML!ul,ll ten on diverti culosis and di d i verticulitis, while o t h ers have begged me to repubr lish on this s u b j e c t be cause it inter ests so many persons. wallet Alvarez X list, 1 should say that many of us phy sicians are under such tremen dous pressure from patients who want a diagnosis of some local disease to explain their abdom inal distress that we are inclin ed to grasp at what I call a diag nostic straw. When we see an old lady whose troubles are all due to a stroke, we examine her carefully and needlessly, as the family expects us to do. If we find some old silent gallstones that never caused any symptoms, we are tempted to say, "Well, there you are. She has gallston es." The family is then much im pressed and happy if we call in a surgeon to have the gallstones out. But this does no good, and often it does much harmi I keep telling professors of medicine that one of the most important courses they could give would be one on recogniz ing findings that have nothing to do with producing the pa tient's symptoms. A name for the course would be the art of not grasping (like the proverb ial drowning man) at straws. Stomach X-Rayed I can remember some 40 years ago seeing an elderly woman who was probably suffering from a high blood pressure with little strokes. Because she had a good deal of distress in her ab domen, as many of these people do, I had the stomach and bow el x-rayed. When I found scat tered along the course of her co lon perhaps 50 little pouches about as big as a pea I was much impressed, and in my ignorance assumed that they must be the cause of the woman's distress. Later, as x-ray studies became common, I discovered that per haps one in three or four of my older patients had dozens of these little colonic diverticula or pouches, and soon I found that they practically never produced any symptoms that I could be sure of. One of my relatives who had over 100 of them strung along his bowel, like a bunch of grapes, lived into his eighties without ever developing any ab dominal distress of any kind. His boweli moved regularly every morning. Hence it is that today I am ashamed of that old diag nosis I made, and when I see a lot of the little pouches, I men tion them only to warn the pa tient never to get frightened ov er their presence and never to take treatment for them. These little pouches form when a person's aging process es cause the muscle fibers cover ing the large bowel to become weak and to split apart. As a re sult, a little of the mucous mem brane lining the colon slips out between some fibers, usually at a point where an artery goes through. Thus a little rupture is formed. Very rarely one of the little sacs perforates, and then there comes an inflammation around the colon. With this the patient gets diverticulitis. Difference in Words Note the difference in the words diverticulosis, meaning a rare inflammatory disease. Unfortunately, we doctors often get these two words so confus ed that we speak of diverticulitis when we mean the harmless and not-to-be-treated diverticulosis. Many of us prescribe diets and medicines, but I never do, any more than I would prescribe a diet for gray nair. All I advise in some cases is a daily enema to keep the affected segment of gut clean. A person with diverticulitis generally gets what looks like a subacute appendicitis on his left side low in the abdomen. He has fever, pain, and local tenderness, and just above the left groin he may feel a sausage-like mass. He may have a little bloody diarr hea or severe consfipation. In such cases if the inflamma- New Deputy Joins Sheriff Office Staff A new deputy sheriff, How ard K. Cramer, will begin work with the Jackson county sher iff's office April 1 as an identi fication officer, according ' to Sheriff Howard Gault. He will specialize in filing, classification and identification of fingerprints and establish a small fingerprint identification office in the courthouse, Gault said. Cramer came to Medford from Portland and has worked with police departments there and in San Diego and Chula Vista, Calif. Cramer replaces Deputy Don Dixon who resigned March 1. There are seven other officers in the department who work on criminal investigation. Gault said Cramer's work would be "a forward step in the sheriff's office investigation setup." It is a difficult operation because many loops of bowel are stuck to the inflamed area, and the field of operation is in fected with bacteria. Sometimes a patient with such an inflamed segment of colon can get by and can ride out two or three at tacks, but if they keep coming, he had better be operated on. Persons Who Lose Much Hair A while ago I said in this col umn that when a person's hair begins to fall out he (or she) should think back to see if, 3 months before, he had a severe illness or accident, or had exper ienced a nervous shock. Now, a woman writes, that just three months after her house burned down, her hair started coming out in combfuls. Now she won ders what she must do. Should she go to a skin specialist or an advertising hair quack? I would suggest that she do nothing. Her hair died three months ago, and the new hairs that have been growing ever since are now pushing the old ones out. All the woman has to do is to wait un til the new hairs grow long en ough to be combed. Dr. Alvarez regrets he hasn't the time in which to read or an swer requests for information.
