We are looking for information from Tribal members concerning services you may need, but currently ARE NOT receiving. We are specifically asking about: • Hearing aids; • Additional funding for orthodontics (braces); • Glasses; • Dental services; • Lasik eye surgery. REFILL LINE: 503-879-2342 If you suffer from: neck or back pain, headaches, have been injured or seek wellness care for your family, chiropractic care may beneÞt you! 17 Important Tribal member Information IBH – Integrated Behavioral Health If you are a Grand Ronde Tribal member with the Skookum Health Assis- tance Program and need to access mental health and/or substance abuse provider services, you need to call Integrated Behavioral Health before accessing those services. IBH has been in place with SHAP for several years now, but it has become apparent that Tribal members are not accessing care through IBH as often as they should. When you call IBH, they will assist you in finding an appro- priate provider for your needs. They will refer you to an in-network provider, which is very important to ensure your claims are paid. Purchased/Referred Care does not pick up out-of-pocket costs when members use an out-of-network provider. Please call IBH at 1-800-395-1616 to coordinate mental health and/or substance abuse services to ensure payment is made. Step 1: Parents will need to notify Shasta at 800-880-5877 or Barbara Steere at 800-749-2928. Once our office has been notified, an Individual Registration Form will be sent to the parents. Step 2: Since 2013, we have asked that all Tribal members apply for state coverage in the state in which you live for the newborn or anyone who does not have primary coverage. Please send in the proof that you have applied for the baby. Step 3: Parents will need to apply for a Social Security number and obtain a birth certificate right away and contact the Enrollment Office to inquire about the process to get the newborn enrolled as a Tribal member. Step 4: Once you have received the Individual Registration Form and filled it out front and back, signed and dated, send it back to the following address: Skookum Health Assistance Program, Attn: Barbara Steere, 9605 Grand Ronde Road, Grand Ronde, OR 97347.
