Although it is Tribally-owned, it in fact is not owned by a Tribal member. We regret the error. NOTICE TO AMEND THE CHOICES OF TAX WITHHOLD ING PERCENTAGES ON 2006 PER CAPITA PAYMENTS During the week of October 1 6, 2006 a Request for Federal Income Tax Withholding from Per Capita Payment was mailed to Tribal members 18 and older, and gave one choice of 28. We have amended the tax withhold ing percentage choices to the following: 28, 25, 15 and 10. Either modify the request form sent to you by writing in your choice of 25), 15, 10 or fill out the modified form below and return to the Tribe. If you would like to have Federal Income Tax withholding from your Per Capita please check the box below. - I want 10 Federal Income Tax withholding from my Per Capita payment. I want 15 Federal Income Tax withholding from my Per Capita payment. I want 25 Federal Income Tax withholding from my Per Capita payment. I want 28 Federal Income Tax withholding from my Per Capita payment. This form must be received in the Finance office no later than Thursday, November 30, 2006 for taxes to be withheld. Forms received after this date will be disregarded.
