At this time, no state income tax withholding is available. A sign-up form for Federal Income Tax withholding is being mailed to all members receiving Elder Security monthly payments. Please fill out and return this form to the Tribe if you want Federal Income Tax withheld from your monthly payment. This service is an op tion, so if you don't sign up, no tax will be withheld. If you have any questions, please contact your tax consultant or the Tribal Elder Tax Consultant, Doris Rose at 1-800-422-0232 ext. 1477 or 503-879-1477. (UMR) as the Tribe's adminis trator for our health benefits program for Tribal members, Tribal Elders, and non-Tribal Elder spouses (age 55 and older). This change will affect where you submit claims and where you call with questions about your claims and benefits. Effective April 1, 2004, UMR will be processing all medical, vision, phar macy and dental claims for members. PLEASE DISCARD ALL ODS MEDICAL AND DENTAL ID CARDS AS OF APRIL 1,2004. Please watch your mailboxes on or about March 22, 2004, for further information and new white plastic Tribal-Logo ID cards (shown below).
