9 APPLICATIONS: Applications may be obtained from your high school couselor or by writing to: Northwest Portland Area Indian Health Board, Health Careers Opportunity Program, 1220 S.W. Morrison, Room 510, Portland, Or 97205. A completed application includes: 1. A completed student application form. 2. An official high school or college trans cript. 3. Two letters of recommendation - one of which must be from your high school counselor. 4. An essay of approximately 300 words stating why you want to participate in this program, and future health career goals and plans. DEADLINE FOR RETURN OF APPLICATIONS IS APRIL 18, 1980. < S I L E T Z * R O L L O P E N E D F O R N E W E N R O L L M E N T SILETZ ROLL OPENED FOR NEW ENROLLMENT - On April 1, 1980 individuals who wish to apply for enrollment in the Confederated Tribes of Siletz Indians of Oregon, should contact the Siletz Office in their area for an enrollment application. The Area offices are located at: Lincoln and Tillamook counties - Siletz Area Office, P.O. Box 549, Siletz, Or 97380, ph. no. (503) 444-2536; Marion, Polk and Yamhill counties - Salem Area Office, 4821 River Road N . , Salem, Oregon 97303, ph. no. (503) 746-9658. $ i i , * ( SILETZ ROLL - To qualify for enrollment an applicant must prove that (s)he possesses 1/8 degree or more Siletz blood. It is the applicant's responsibility to provide the necessary documents for proving blood degree. Some of the documents that an applicant will need or may provide along with a completed application, are: 1. a birth certificate (showing mother and father and mother's maiden name); 2. Marriage license; 3. Bible records; 4. Baptismal or other church records. After receiving a completed application and supporting documents, the enrollment staff will make a preliminary recommendation to approve or disapprove the application. That recommendation must be made to the Tribal Council within 60 days from the enrollment staff's receipt of the completed application. Staff may request additional information from the applicant. An applicant has 30 days to provide such additional information. Applicants recommended for approval will receive a certified letter stating the staff's action. A copy of that letter will be posted in all Area Offices for T r i hal members bo read. If any Tribal member believes the Enrollment staff was wrong in recommending approval aPPlication, (s)he may file a written protest with the Enrollment Secretary, Tribal Office, P.O. Box 549, Siletz, Oregon 97380. That protest must specify why the staff's recommendation was in error and must be filed within 45 days from the posting of the staff1g reccmnendation in the Area offices. Enrollment staff will contact the applicant, if neces sary, to ask for additional information in light of the protest. The Tribal Council shall hold a hearing on the protested application or any application approved by the Enrollment staff which the Tribal Council decides to reject. If the Enrollment staff recommends that an application not be approved, the applicant will be notified by certified mail and has 60 days to appeal the staff recommendation. All appeals must be supported by documents, records, or other evidence showing why the staff was wrong to reject the application.
