Fragassi P# 188279 Publish: 02/05, 02/12. 02/19, 02/26, 2003 Health care workers to be vaccinated to prepare for outbreak Vaccination of Oregon’s public health and health care workers in prepara- tion for a potential smallpox outbreak will likely begin in mid-March, accord- ing to state Dept. of Human Services (DHS). “Of course, if a smallpox case oc- curred tomorrow, we would begin vacci- nations immedi- ately,” said Mel Kohn, DHS state epi- demiologist. “In the mean- time, there is no out- break and there are unresolved legal questions. We need to finalize and prac- tice vaccination clinic procedures. And, every health depart- ment in the state is in the midst of making sure kids have all their shots before school exclusion day on Wednesday, Feb. 19,” said Kohn. Section 304 of the Homeland Secu- rity Act went into effect Friday, Jan. 24. It contains federal liability protections that are intended to clear the way for the national smallpox vaccination program to begin. “We still need answers to questions that county health departments and hos- pitals are asking. They are concerned about issues related to workers compen- sation, liability and loss of pay in the event of serious ill- ness ore even death resulting from the vaccine,” Kohn said. “Right now, legal protections for those giving the vaccine and those receiving it who may become ill, or event the contacts of vacinees who could become ill are not clearly defined,” he said. Kohn said the risk of severe reac- tion to the vaccine is an important consid- eration. “Thirty years ago, before smallpox was eradicated, every citizen was routinely vaccinated. One in 25,000 persons vacci- nated had a life- threatening reaction to the vaccine and one in a million died,” Kohn said. The smallpox vaccine program was halted in 1972 be- cause of concerns about the safety of the vaccine and be- cause the disease was no longer a threat, according to Kohn. In December, Oregon submitted two plans for con- fronting a possible smallpox outbreak to the federal Centers for Disease Control and Prevention. One plan calls for vaccinating approxi- mately 800 state and local public health and private hospital workers around the state against small- pox. These vacci- nated health workers would be able to in- vestigate potential cases, give vaccina- tions and care for potential smallpox patients. The second plan, which calls for wider vaccination, would be used only if an actual smallpox case were discovered. “ Ea r lie r t hi s month, we trained 70 public health staff on how to do a smallpox vaccination.
