Siletz Clinic’s No-Show Policy means
arrive on time for your appointment
The Siletz Community Health Clinic
has a No-Show Policy that went into effect
Feb. 17, 2017.

for medical appointments. The Walk-In
Clinic is available first-come, first-serve.
When does a no-show happen?
Patient will be required to utilize the
walk-in clinic for 90 days, after which
patient will be able to return to scheduling
appointments.
A no-show happens when a patient
does one of the following:

Does not arrive to his or her appoint-
ment
Cancels the appointment with less
than 24 hours’ notice
Checks in more than 10 minutes after
the scheduled appointment, which
results in the provider not being able
to see the patient

patient doesn’t show up for an
appointment?

The health and all treatment for the
“no-show” patient is impacted
The health and all treatment of
another patient who could have been
seen in the appointment slot
Waiting times and subsequent treat-
ment for the rest of the patients on
the schedule for the day
The providers’ limited time and
resources are wasted because they
prepared for an appointment that did
not happen
What happens if a patient is a
no-show?
Medical, Lab and Radiology Services
First No-Show – After the first no-
show, the patient will receive a letter and
a copy of the policy for review.
Second No-Show – Patient will
receive a letter providing notice about
Third No-Show – Patient will receive
a letter providing notice about the three
no-show appointments and their require-
ment to utilize the Walk-In Clinic for
medical appointments. The Walk-In
Clinic is available first-come, first-serve.
Patient will be required to utilize the
walk-in clinic for 180 days, after which
patient will be able to return to scheduling
appointments.
Dental and Optometry Services
First No-Show – Patient will receive
a letter providing notice about the no-
show appointment and their requirement
to utilize the department’s Walk-In Clinic
for appointments. Walk-in availability is
first-come, first-serve.
Patient will be required to utilize the
Walk-In Clinic for six months, after which
patient will be able to return to scheduling
appointments.
How to avoid getting a no-show!

Confirm the appointment.
•
Arrive 10-15 minutes early.
•
Give a minimum 24 hours’ notice
when canceling appointments

Greetings:
I would like to take this time to recognize my son, Isaac (Ike) Butler (pictured above),
a senior at Siletz Valley Early College Academy. In February 2018, Ike became the
first Oregon School Activities Association (OSAA) state wrestling champion in Siletz
Valley’s history, with a first round pin in the final match of his high school wrestling
career. He wrestled in the 182 pound weight bracket for the 1A-2A schools.
Ike is also the school’s first four time district champion. He is a three-time 1A state
champion and placed third at the OSAA championships as a junior. During his sopho-
more and junior year, Ike qualified for Oregon’s national wrestling team and recently
qualified for Oregon’s international team. Ike finished his high school wrestling career
with 107 wins.
My son was also selected to play in the Oregon 8-Man All-Star Football game and
was nominated for the Oregon East West All-Star Shrine Football team.
Ike plans on furthering his education and wrestling at the collegiate level.
I would like to thank all the Warrior wrestlers and their families for the memories
and support of our wrestling team, the coaches for their commitment to my son, the
Siletz community and the Siletz Tribe. Warrior Heart!

To the editor:
I believe that tiny South Beach Cemetery should be taken into trust by the Siletz
Tribe for tender loving care. Several of our Tribal ancestors are at rest there.
Some signs of past vandalism have already occurred. The county has given only
minimum care in the past. Thanks to the county for what they have done, but I have to
believe the Tribe can do a better job of preserving this cemetery.
Proper care with security cameras is a must for this little speck of sacred ground.
