Missing, continued from page 1 The Tribal Member Information Change Form can be obtained in three ways: 1. The fastest way is to print the form from the Tribal website. Go to ctsi.nsn.us; at the left of the home page under Government Listings, scroll to Enrollment and the form is on the right side of the page. 2. Stop at any Tribal office in Eugene, Portland, Salem or Siletz where the form can be provided to you. Fill it out and ask the staff to email and mail it to the Enrollment Department. 3. Call the Enrollment Staff at 800-922- 1399, ext. 1258, or 541-444-8258 and request a form be mailed to you. Even though you will need to provide a new mailing address, that address will not be used to update your address with Enrollment. Enrollment will only update your address from the form we receive. Deadline for submissions of art to be considered is July 10. Winners will be determined by July 15. First place ($200), second place ($75) and third place ($50) win- ners will be selected. Artists must have a Tribal enroll- ment number from a federal or state- recognized Tribe. The art chosen for first place will be used for publicity and marketing of the conference on such items as T-shirts, bags and conference books. All submissions will become the property of CANAR, Inc. Entries must be two-dimensional artwork created in any medium, such as painting, drawing, digital, collage, photography, etc. The work can be expressed from realism to abstraction.
