The Tribal Member Information
Change Form can be obtained in three ways:
1. The fastest way is to print the form
from the Tribal website. Go to
ctsi.nsn.us; at the left of the home
page under Government Listings,
scroll to Enrollment and the form is
on the right side of the page.
2. Stop at any Tribal office in Eugene,
Portland, Salem or Siletz where the
form can be provided to you. Fill it
out and ask the staff to email and mail
it to the Enrollment Department.
