Back to School Night and Resource Fair Siletz Valley School Aug. 30, 2012 LIHEAP ready to provide energy assistance to households this winter • What is LIHEAP? LIHEAP (Low-Income Home Energy Assistance Program) is a federally funded program that helps low-income house holds with home energy bills. LIHEAP can help you stay warm in the winter, which can help you reduce the risk of health and safety problems, such as ill- ness, fire or eviction. What assistance does LIHEAP provide? * • • • Heating assistance Crisis assistance (shut-off or final notice) Who is eligible? 8 Enrolled Siletz Tribal member o The applicant or a child residing in the household must be an enrolled Siletz Tribal member. • Siletz News • Required documentation includes, but is not limited to: • Social Security cards for all house hold members (no exceptions) • Most recent utility bill • Income documents for the previ ous year for each adult household Oct. 1 - Siletz Tribal Elders and disabled Tribal members Nov. 1 - Other Siletz Tribal households What information do I need to apply and what is different from previous years? • • LIHEAP is divided into two catego ries: 1) Heating; 2) Crisis. The maxi mum award is $500 per household. Not all families will qualify for the maximum amount. Awards will depend on family size, income and need. When making your appointment for assistance, please be sure you will have all necessary documentation before your appointment time. October 2012 member (e.g., Social Security award letter, unemployment printout, pay stubs, etc.) If you need documentation from other departments, please be sure to make those requests ahead of time. Siletz Service Area Only When can I apply? • Individuals who meet the require ments below are eligible for heating or crisis assistance. • • Residence location o The residence must be located within the 11 -county service area. Income eligible o The applicant/family must meet the annual income guidelines as determined at the time of intake.
