Siletz Community ^7 Clinic Health CHA-MAY WEEYA - MEDICINETALK Notice of Applied and Awarded for Housing and Urban Development Out-of-Area Health Benefits Indian Community Development Block Grant Funds (ICDBG) for the Construction of a Tribal Fitness Center Facility by Judy Muschamp, Health Director The next call-in date for out-of-area health benefits is July 3, 2006, begin ning at 8 a.m. The number to call is 1-800-628-5720 or you can try the local number, 541-444-1236. Staff will not begin an swering the telephone until 8 a.m. and voicemail messages will not reserve funding. This is the first quarter for the 2006 excess pledge revenue funds; $200,000 will be available to disburse. Each caller is eligible for one of following: This notice is to serve as the required Community Development Statement Request for tribal member input The Confederated Tribes of Siletz Tribal Council has authorized the preparation of an application for fund ing under Housing and Urban Development’s Indian Community Development Block Grant program (ICDBG). The following is a list of eli gible activities, threshold requirements, and rating factors. Eligible Activities • • • • Program-related threshold require ments: Assurance that the tribe has no outstanding ICDBG obligations. a. b. c. Project-specific threshold require ments: Rating Factor No. 4: Leveraging of resources Rating Factor No. 5: Comprehen siveness and coordination a. b. Improve housing stock Provide community facilities Improve infrastructure Economic development Under public facilities and im provements, there are no project specific thresholds. Under public service projects, there is a 15 percent cap on the amount of grant funds to be used for this component. Rating Factors Possibilities • • • • • • Remodel of closed buildings for public program use (not govern ment) Housing Department community office Clinic expansion Water and sewer infrastructure for housing subdivision Real property acquisition Construction of a special needs or health and wellness-specific facil ity Deadline for submission: Aug. 25, 2005 Estimated award date: Jan. 1. 2006 Ceiling amount: $500,000 Rating Factor No. I: Capacity of ap plicant requires a minimum of 15 points a. b. c. d. e. f. Management of project Technical staff experience Project implementation Financial management Past performance Procurement and contract manage ment experience Rating Factor No. 2: Need or ex tent of problem Rating Factor No. 3: Soundness of approach Project design Budget and costs Sustainability The results of this request for com munity input will be evaluated in con junction with the results of the com munity meetings held in 2004 and the cumulative results of the tribal-wide survey and produced 2004 l()-year comprehensive plan for the tribe. The entire Community Develop ment Statement is available for public viewing at the Siletz Tribal administra tion offices in the Planning Department. It will be available for review after June 20 from 9 a.m. to 4:30 p.m., Mon day through Friday. Note: Subsequent to approval by Tribal Council at the August 2005 Tribal Council meeting, this document will be placed in the tribal newspaper and the Newport News-Times as per grant requirements. $400 Vision $1,200 Dental $1,400 Hearing $1,000 Medical Once you receive authorization, you must use the benefit within 90 days. If benefits are not used, the money then becomes available for the next call-in date - Oct. 2, 2006. If you have a prob lem, please call Danise Barker in the Contract Health Service office before the end of September 2006. Other considerations: An indi vidual can access benefits only one time per year. When you call for pre authorization, please remember that CHS will authorize for household mem bers only. Authorization will not be granted for anyone not residing in your household. Caps • • • 15 percent cap on amount of grant funds that can be used for public services 20 percent cap on planning and administration 10 percent cap on technical assis tance for development of capacity to undertake project Thresholds Thresholds: Minimum 70 percent of grant to be used for activities that ben efit low- and moderate-income persons and be in an activity which, if it ben efits all residents, at least 51 percent must be low- and moderate income.
