Cha-may weeya Medicine Talk Siletz Community Diabetes Exercise Program Applications Due July 1 -15,2003 - Enrolled Adult Siletz Tribal Members Program info note: Participants admitted to the program and meeting their attendance and documentation requirements don’t need to submit a new application to continue this program. Applications will be accepted for slots that remain open or become open. Please note that a very limited number of slots exist and that submitting an application does not guarantee accep tance into the program. It’s to your advan tage to complete your application fully. Please see application materials for pro gram details and our priority populations. They have been working on it since December 2002. The premier will showcase a 15- minute video developed by the Siletz group, an interactive Web site developed by the Eugene group, and a spectacular brochure and poster designed by the Salem group. Join us at the tribal t r community center in 2^7 * Siletz at 7 p.m. Light refreshments and fresh popcorn will be served.
