The SSN will be used by the district fo r reporting, re search, and record-keeping. Your SSN w il also be provided to the Oregon De partment of Education. The Oregon De partment of Education gathers informa tion about students and program s to meet state and federal statistical report ing requirements. It also helps school districts and the state research, plan, and develop educational programs. This information supports the evaluation of educational programs and student suc cess in the workplace. The school district and Oregon Depart ment of Education may also match your SSN with records from other agencies as follows: The Oregon Department of Edu cation uses inform ation gathered from the Oregon Employment Divi sion to learn about education, train ing, and job market trends. The in formation is also used for planning, research, and program im p ro v e ment. State and private universities, col leges, community colleges, and vo cational schools use the information to find out how many students go on with their education and their level of success. Other State agencies use the in form ation to help state and local agencies plan educational and train ing services to help Oregon citizens get the best jobs available. According to Federal and State law 'Each school district must establish pro cedures which ensure that all children re siding within the jurisdiction of the school district who are disabled, regardless of the severity of their disability, and who are in need of special education and re lated services are identified, located and evaluated, including a practical method of determ ining which children are cur rently receiving needed special educa tion and related services and which chil dren are not currently receiving needed special education and related services.* P ersonnel in C olum bia C ounty ask that you notify the contact person in your school district resident area, the Head Start office, or the Early Intervention/Ear- ly Childhood Special Education Intake Coordinator for the County if you have the name of a child suspected to have a disability.
