A few hours later he wrote his first case note, In pencil In hi per sonal notebook: "T doctor "First noticed vision Incoherent or something at 1M0 (4:40 p. m.) cant even see dearly what I'm writing. "If anybody puts me In a respir ator please note: I cannot swallow. I have a rapid accumulation of mucous In my throat. If I am not allowed to turn my head to the aide to spit I'll choke to death. His next brief not waa an hour later: "Doctor -175J (5:53 p. m.) noted Tery poor control of tongue In mouth." Mrs. Chiles asked him how things were going, but he could only answer In a scrawl: "My guess If that I'm going to be an right." She asked If he were In pain. He replied by pencil: -Some but not bad. "Every person on this floor has a (word unreadable) contagious dis ease dont want you around. "Cant read vision's shot tempor arily. "Only thing I'm worried about Is you. And then waa the last not of all: "My neck la paralysed, doesn't hurt anymore." That was about I p. m. Wednes day. Lieutenant Chiles died, of Bulbar Poliomyelitis, at :20 the next morning.
