At the Mayo Clinic, between 1953 and 1957, inclusive, tran surethral operations (performed through the tube which carries urine from the bladder to tne outside) were performed on 23,938 men. The mortality after this operation was only 1.04 per cent, which is mucn lower tnan that of any other prostatic oper ation. Usually, with this opera tion the patient is up and walk ing around within a tew hours. The suprapubic (the public re eion is the front part of the pel vis) operation is performed for the removal of very large pros tate elands, such as those that weigh more than three ounces. Another operation is the retro pubic (behind the public region) and another is the p e r i n e a 1 (through the crotch). In all these three operations a big cut has to be made through tne SKin. The Mayo urologists feel that with adequate training and ex perience a man should be able to remove the obstructing part of a large prostate gland throueh the urethra in more than 90 per cent of the patients. Myasthenia Graves Because quite a few anxious people ask me if there is any thing new in the treatment of myasthenia gravis, I will say that in the New England Jour nal of Medicine for March 28, 1963, Dr. Robert S. Schwab, of the Myasthenia Gravis Clinic in the Massachusetts General hos pital in Boston, spoke favorably of the use of Mestinon Time span, which can give a sustain ed effect for from six to eight hoursJSphedrine may help, also Protopan and Tensilon; ACTH appears to be of doubtful value. All drugs must be used with care because an overdose can throw the person into a crisis. The dose has to vary with the needs and degree of sensitive ness of the individual patient. Recently, Drs. Kermit E. Os- serman and Gabriel Genkins of New York, discussed 47 crises (flare-ups) which they have had to treat in 35 patients with "lis rare disease in which the per son's muscles become very weak. Up until some years ago, such crises commonly caused death, perhaps because the pa tient would become too weak to breathe. Today there are sev-l eral drugs commonly used to help drugs such as neostig mine bromide, neostigmine methylsulfate, pyridostigmi n e bromide, and ambenonium chlo ride. With medicines, many pa tients can now be saved. More Dangerous The small thymus gland in the chest is abnormal is about 85 per cent of all those cases in which it can be examined. Sometimes there is a tumor in it (called a thyoma), and this may make the disease more danger ous. Dr. Schwab teels tnat all the tumors of the thymus gland should be removed surgically, if the patient is strong enough to stand the procedure. Other surgeons believe in removing them only in certain cases. X-ray treatment before removal of the tumor increases the chances of success in helping the patient. About half of the 60 patients who, according to Dr Schwab's report, had this treatment were helped, women under i years of age, who have had the dis ease for less than five or six years, who have had the thymus gland removed, and wno aid noi have a tumor in it, have a 35 per cent shance of becoming ap- oarenty well. The operation worked better for women than for men.
