R 1 mm 9 V Lymphedemi of the Legs Drs. Alexander Schirger, E. G. Harrison Jr., and J, M. Janes o the Mayo Clinic i-gw have reported a stuay 01 jji cases of what is called lym phedema o( the legs. This 3 is a disease in which one or both legs be come enor mous, because Alvares ' Df the accumu lation in the tissues under the skin of an excess amount of the fluid which normally bathes and nourishes the cells of the body. In many Islands of the South Seas the disease is call ed elephantiasis, and is due to the introduction Into the tis sues (by mosquito bites) of a tiny worm which obstructs what are called tne tiny ivmph channels. Fortunately, in this country we haven't this form of the disease. Here, lymphedema seems often to be due to some inborn aeicci in the construction of the lymph channels. A woman can quickly tell if her leg swelling Is due to lymphedema or to dropsy due to a bad heart, bad kid neys or perhaps bad veins. All she has to do is to press her thumb against the swollen tis sue. If for a moment It leaves a little pit or indentation, it is probably a dropsy (edema) while if it does not pit, it is probably a' lymphedema. In 123 of the 131 cases re ported, the disease started ear ly In life, and was probably due to some congenital abnor mality In the lymph vessels. This type of lymphedema of ten appears early In the per son's teens. Casts Fairly Hare There is another rarer form of the disease which comes after the age. of 35. Only when the disease comes after the age of 40 or 50 does one ex pect that a lymph channel has been obstructed, as perhaps by a cancerous growth. Such cas es are fairly rare. In all but a tew nf thrlr cases, the Mayo physicians treated the patient with only a slrong elastic support. But a number of women, for one reason or another, gave up the use of this mpport. There is an operation In which the thickened, swollen tissue Is cut out and removed. At the Mayo Clinic during several years, only nine of these operations were per formed with good results In eight cases. Usually, I find surgeons re luctant to perform this opera tion, perhaps because there Is often a good deal of shock to It. and the results can be un satisfactory. As one can easily see, when the surgeon had to remove all of the skin from , the enlarged leg, then all of . the swollen tissue, and then j must try to cover the huge wound with skin, it is a big and dangerous undertaking. The Mayo surgeons operat ed only in those cases in which the size of the limb was increasing rapidly in spite of continued medical treatment; or when the function of the j leg was so impaired as to make walking difficult; or when troublesome changes were taking place in the skin of the leg, or when the size of the limb caused it to be unsightly. Much depended on whether the person was old or young. Naturally, a young woman was far more distress ed by an unsightly limb than an old woman. I have seen some of these young women who have said to me that they would rather die with an oper ation than to go on living with the dally humiliation caused by their great big legs. An Important point that the women should know Is that in these cases, removal of somewhat enlarged veins doesn't do any good. I have seen a number of these pa tients who were no better for having some veins removed. At the Mayo Clilic, a band- 1 age made of 100 per cent rub ber has proved to be the most satisfactory form of support. Calcifitd Pineal Gland Some people get worried when an x-ray man tells them they have a calcified pineal bland, a gland located low within the brain. Many men have tried to demonstrate that the tiny pineal body makes a secretion that is valuable to man. It makes a secretion that has some influence on the sex glands, but just what this does for a person is not yet clear. If an x-ray film showed that I had a "calcified pineal" I would not be at all concerned. In rare Cases, a tumor forms in the pineal gland one that may have to be removed surgically. It may cause a boy to develop sexually in a pre cocious way. The disease Is so rare that one should not wor ry about it.
