MEDFORD MAIL TRIBUNE. MEDFORD. OREGON TUESDAY. JULY 24. 1962 B 11 ff-' ,! "'S3' . -to,- I i A WATER BIKE DISPLAYED - Taking the center of the stage this week at Savage Rapids on the Rogue river was this water bike designed and built by Bob Rausch of Rogue River. The builder says the idea is not new, but thii is his adaptation of the The Medkal Swelling of Arm After Breast Removal Because hundreds of wom en write asking me what to do for an arm that has become badly swollen after the re mo v a 1 of a breast for can cer, I am glad to find in a recent num ber of the Journal of the American Med ical As sociation, an R. C. Britton, Alvares article by Drs. and P. A. Nelson, of the Cleve land Clinic, who made a study of 114 patients with what is called lymphedema of the arm after a radical removal of the breast. It cheers me much to see that in 94 of these patients there was no local recurrence of the cancer. That was not the cause of the swelling -the cause that the physician often hopes against hope is not present. The study made by the doc tors suggests that infection, either entering the tissues of the arm right after the oper ation or at some later time, is the most important factor in producing the swelling. Oc casionally, the surgeon can see whence the infection came - perhaps from an abrasion of the skin, or from an infec tion of the edges of the sur gical wound. In most cases during the operation the surgeon leaves Intact enough tiny channels (lymphatics) for the passage of lymph (a fine fluid which bathes the body cells) and larger channels-veins - which carry venous blood back from the tissues to the heart. The surgeon removes the lymph nodes in the armpit in an ef fort to get rid of all possible cancerous tissue that may be hiding there. As I said, usual ly he leaves enough lymph atics to keep the arm from swelling, but sometimes he may either take out too many of these little channels, or tome inflammation may come along to close up the ones he left. In a rare case a vein can become plugged up with a blood clot. Literature Revitwed Drs. Britton and Nelson re viewed much of the literature, which shows that usually in They'll Do It Every I 7 t JO' "W V W l K s r f , - asv --, -r SWT ' principle and ihis is me fnsi uuiuductuin in Southern Oregon. A number of the surgeons who have studied this problem in the past have concluded that a slight chronic Infection after operation was the most im portant factor. Other surgeons have sus pected that the x-rays which were used after operation had much to do with producing the obstruction of the lym phatic vessels, but Cleveland surgeons found that in half of 94 cases the swelling appear ed in the arm of a woman before she had x-ray treat ment. Several writers have con cluded that probably in some cases several factors work to gether to produce the swell ing of the arm - factors such as infection, the use of x-rays, and the obstruction of veins. X-ray studies of veins have shown that usually they are not at fault. In the way of treatment, doctors have advised eleva tion of the arm, massage, hy drotherapy (with warm wa ter), exercises, and mechanical compression with elastic ban dages. Unfortunately, these measures have seldom achieved lasting control of a severe lymphedema. Also, these methods are not prac tical for a person who has to work all day, and usually they soon are abandoned. Results Not Promising Surgeons have tried to make new plastic channels for the lymph through the affect ed arm, but the results of these operations have not been promising. More success ful have been certain proce dures at the time of operation, which appear to have caused new lymphatic vessels to der velop. As I said, the two Cleveland doctors have treated 114 wom en, ranging in age from 25 to 72 years. All of them had from moderate to severe lymphe dema of the arm. A number of them suffered limitation of motion in the shoulder, and i an effort was made to limber bles and Goitrrs ' To obtain up this joint.
