Many say the Doc tor just told them to go to the hospital where he would oper ate, but did not explain why he wanted to operate, or what he planned to do. The patient could not find out if the operation was urgent or could be put off for a while, or if it was dangerous to life, or if it was likely to be suc cessful. Could there be a poor or unsatisfactory result? Fin ally, the patient could not find out even if the doctor was a surgeon, or if he was competent to perform the type of operation that was needed. For instance; I have a let ter from a young woman who, after a difficult labor, loses an embarrassing amount of urine whenever she coughs or sneezes or even walks down stairs. The physician consulted said, "Go to the hospital, and I'll operate," and that is all the family could learn from him. After some talking with friends the family wonders if perhaps they should see a urologist (bladder specialist). Other friends think the work should be done by a gynecologist. They so wish their doctor would talk to them long enough" to answer all of their questions, and they so wish they could have a consulta tion with some wise and kind ly old doctor who would' tell them what he would du for his wife if she needed a tight ening - up of the outlet of her bladder. Often Difficult Actually, I talked about this subject with one of America's ablest gynecologic surgeons and he told me that the operation is often a diffi cult one; and even with all his 35 years of experience, he cannot always get a perfect result. He might perform a peri neal repair, bringing the torn muscles together and pushing the bladder up where it be longs, and would hope that this would give a good enough result. Then I talked to a professor of gynecology in a great uni versity school of medicine, and he said, "f never operate on a woman like that until I had her and perhaps her hus band in the office for one or two good chats. I want to get acquainted with her first, so as to size her up as a phycho logical risk. I don't want later to find out that I have oper ated on a psychotic woman who will perhaps hate me and even sue me; and second, I want to answer every ques tion that is in her mind. As a result of this type of friend ly behavior with his patients, this surgeon is much loved, and he has always had a huge practice. Afraid of Feelings A point I must insist on is that many people, when they want help, are too afraid of hurting their doctor's feelings, or they are too afraid of breaking some rule of eti quette. They do not know that when they want a con sultation, perhaps in order to get answers to all their doubts and questions, they should ask for it, and insist on it. If their doctor refuses them or gets stuffy about their re quest, they have a perfect right to leave him and go to someone else. Many a patient has said to me, after an unhappy and un fortunate experience with a consultation, "I did not want a consultation I wanted to leave my doctor, but I did not know how to do this." Actu ally, I feel that every patient I sec is a free agent, and if he does not like me, all he has to do is to quit coming to see me. If I were calling on him in his home, he could just say, "You need not come any more." If he is in a hos pital, the situation may be more dificult, because hospi tal authorities hale to have a patient change his physician while he is in the place. But he usually can get to another hospital, and another doctor. Not As Fussy Today, sensible physicans are not as fussy about so called ethics as tlicy used to be, and when one doctor did everything for the members of a family from grand father to grandson. Today a doctor Just can't be stuffy. Why? To Illustrate: I could not have the happy relations I have with many of my subur ban patients if I were to get angry over the fact that al though the head of the house sees me twice a month about his high blood pressure or his stomach ulcer, when he has a flare-up of his gout he sees the arthritis expert to whom I once sent him; when his back is aching, he sees his osteopath; when he has acute bronchitis, he calls in the able general practitioner in the suburb where he lives; when his wife is ill she sees her gynecologist, and when his children are ill he calls in their pediatrician. In the last half century the practice of medicine has greatly changed, and a wise doctor faces this fact. To sum up then: when a person wants a consultation and needs it, he should insist on having one. If he wants to change doctors he must not start with a con sultation; he must make a. clean break with his doctor. The scientific facts about overweight are enough to make the jolly fat man grim. Dollcr, pro fessor and chairman of the department of radiology, Uni versity of Oregon medical school, will speak at a meet ing of the Jackson County Medical Society at 7 p.m. Wednesday, March 8, at the Rogue Valley Country club.
