on what pub

city. On inves

high percentage of their two
or three hundred clinical
laboratories were doing Inac
curate work.
Some of it the authorities
said wus disgraceful. This
only reenforces what I have
been preaching all my medical
life - that we doctors must
not try to get nil our diagnoses
made for us by a laboratory
girl somewhere. She could be
wrong, and hence we must
keep thinking about our pa
tient's problems; we must look
at him or her and wc must
take an adequate history.
But to get back to the prob
lem of the woman with the
supposed thyroid disease.
Usually in these cases I make
my diagnosis, not In the lab
oratory, but by looking at the
woman and talking to her. If
she is decidedly hypothyroid,
she is generally overweight,
slowed-up physically and men
tally; she may have gone to
sleep in the chair in my reception-room:
her skin may
be very while and cold and

a fur coat on a warm summer
day; she may say she piles
the bed-clothes on at night;
she may have quit menstru
ating, and she may have lost
all interest in sex.
She may say she feels as if
she were thinking through a
heavy veil. With such a story
I hardly need to have a test
made of her thyroid gland. I
give her several grains of
dried thyroid substance a day
and if, in a few weeks, she
has greatly changed in ap
pearance; if she feels bright
and wide-awake again, and if
hor husband says that at last
he has a wife, I am sure my
diagnosis was right.
Similarly, when a thin
nervous woman comes in with
overly prominent eyes, per
haps a small goiter, warm
feverish, trembling hands, a
fast pulse, and wide perspir
ation stains in the clothes in
her arm pits; and when she
tells me that she feels jittery
and so warm that she is al
ways opening windows and
kicking off her bed-clothes;
also, that she has been losing
weight, and having trouble
getting to sleep, also, that her
husband complains that she
has become "ornery" and un
reasonable, I don't need a
laboratory girl to tell me that
the woman has a nuich-too-active
thyroid gland.
