These, I gen erally cannot answer be cause I do not know whether the pa ticnt has the acute, I self - limited form of the disease, or the often puzzling chronic form. Also, no one knows the cause of the di sease, and this is a great handicap when it comes to treatment. Hence often we doctors are a bit amusing in our behavior. As in this case, where some of us describe a disease in Greek or Latin terms we think we know what it is. "Idiopathic" means we don't know the cause. Throm bocytopenlc means a lessen ine of the number of throm bocytes (blood platelets which are tiny bodies about which the blood clots) and purpura means a condition in which purplish patches form in the skin or the mucous mem branes - due to the escape of blood into the tissues. As I often say to medical students, "Never think that by describing a disease in Latin vou have learned about its cause or anytmng arjout curing it. You are just as ignorant as you were when you started." Puzzled About Nature Even Dr. William Dameshek of, Boston, one of the world s experts on diseases of the blood is puzzled about the nature of purpura. As he says, there is the acute form which may follow an attack of chic ken pox or measles or a virus Infection, or some type of vaccination, or the taking of some strong drug. Then, most doctors advised removing the spleen (a solid organ on the left side, next to the stomacn); today, they like to wait, be cause many a patient will get well anyway, without much treatment. Even in chronic cases of purpura Dr. Dameshek now waits a long time - even year and a half - before he feels like ordering a splenec tomy( removal of the spleen). A complication is that in some cases the disease turns into lupus erythematosis, another puzzling condition with ab normalities in the arteries, heart, kidney and skin. Which reminds me that hun dreds of people with some disease write asking if I think it wise for them to make financial sacrifice and travel a thousand miles or more to some big clinic or medical school somewhere. When the disease is a common one like hardening of the arteries or a stroke, or a heart attack or psoriasis, I say, "Better not go. Your fine doctor at home knows perfectly well how to take care of you. But when a man writes saying, "I .have condition so strange and mixed - up and exceedingly rare that no doctor in my town has ever seen a case like it; and cannot be sure what it is," I say, "If you were my brother, I'd take you to a great medical center where I would find a few doctors who would say, right off, 'Yes, we've seen dozens of cases like this, and we arc beginning to understand the problem, and sometimes to stop the course of the disease.' Usually, because of their great experience, these doctors can say, 'This is not lupus ery thematosis; It is not infectious endocarditis, and it is not a beginning leukemia,' and that it helpful." In a recent number of the Journal of the AMA, an ex. pert says that in the treatment ox a chronic sinusitis, the nose man locates the infection; per haps finds what sort of a germ is at work; he washes out the pus, and if the drain age is poor, he may cut a little window into the sinus. Espec ially when the pus is in the antrum, (hollow place in the cheekbone) the window he makes may quickly work cure. Occasionally, the doctor will straighten a deviated sep tum (bone down the center of the nose); he may remove polyps - little soft jelly-like tumors which obstruct breath ing, and he may trim off a bit of an obstructing turbinate bone (a tiny shelf on the side of the nasal cavity.) Ozena, or Bad Smelling Nose There is a miserable disease fortunately rare (in my ex perience) in which the inside of the nose becomes full of foul smelling "crusts." A num ber of people ask me about it; all I can say is "go to a good nose specialist and see if he can help you. Perhaps he can clean up your nose."
