Here, for instance, comes a letter .from a woman who says she has a goiter. Her doctor wants to take it out, but she does not want to con sent to this until I give her some advice. But I cannot give any, because there are all sorts of goiters that present different problems. How do I know if the goiter is harmless, or how do I know if it is so highly toxic that it should quickly be removed surgically, or perhaps treated with radioactive iodine? Per haps the goiter is pressing on her wind-pipe, or it is serious ly damaging her heart, or her nervous system. Perhaps she tells me that the doctor wants to operate because he has re ceived a report that she has too much iodine in her blood. But how do I know that this abnormal report is not due to some medicine she took recently? Can't Give Opinion Dozens of men say that their doctor wants to remove their prostate gland; now they want to know whether I think they should accept the opera tion. Again, how can I say anything when I don't know even if the man is at all bothered by his prostatic en largement? I cannot give an opinion unless I know how much residual urine he has -that is how much urine re mains in his bladder after he thinks he has voided com pletely. A dozen women ask me what they should do about their menopause. Should they consult a psychiatrist, or should they be taking ovarian extract, or getting electro shock treatments? Again, How can I tell? I don't know how ill the woman is. Some women of 50 just stop menstruating, and that's all. They have no distress. Some have mild flushes, some have severe flushes, and some become much depressed. Some are de pressed, not so much because of their menopause, as because of anxiety . over their hus band's heart attack, or his stroke, or. their daughter's divorce and return home with two children to be taken care of. Cataract Questions , . Every week I get letters from people who say that they have a cataract in one eye, and some doctor is keen to take it out. They want to know what I think. Mayo Cltnle is. If the other eye is perfectly good, I could write them quot ing dozens of people who have written me, saying they wish ed to God they had not been operated on for one cataract. Before operation, they could read comfortably, and now they can't. The vision in then two eyes is so different that they can't focus the two to gether. As a result, they have to wear - an unsightly patch over one eye. I have the same difficulty with dozens of people who write to ask me whether they should take strenuous treat ment for a high blood pres sure. Some say they tried the drug the doctor gave them but they felt worse when tak ing it. Again, I cannot advise because perhaps my corres pondent is a woman of 65, with a so-called high blood pressure of 160 mm. of mer cury. If I were to talk to her I would find that she hasn't any symptoms, at all, and hence, to my way of thinking, her so called' high blood pressure should be about as dangerous to her as her gray hair. To take away all her salt and to give her some powerful medi cine which might make her very ill doesn't seem to me to be very good .medicine. Pain in Back As I write this, my secre tary hands me a letter of a type which I cannot possibly answer.. A woman of 50 says she has a miserable pain in her back. Her old family doc tor, who knows her well, thought it was due to nervous ness and fatigue, and told her to get some rest. Not liking his opinion, she went to a man who said she had a rup tured intervertebral disc and should be operated on for it. Not liking this either, she went to another man who said it was due to a large left ovary. Now a psychiatrist says that her trouble all the while was a mental depression, and she ought to have shock treat ment. My impression from the letter is that the psychiatrist is right, but how can I do any guessing about the diagnosis? If I could see the woman, I might pot hazard an opinion until I had quizzed her and perhaps her husband and daughter for at least four in terviews of a -half-hour each. The big point is that when a person has a problem of diagnosis that has stumped three or four doctors at home -who could question at length, and then examine carefully it is useless to ask me to guess what the disease is, without giving me any chance to ques tion or to examine.
