About 100 men were idled at the Air Force base when a two-man picket line was established this morning. The Medical by Hepatitis and Cirrhosis of the Liver I get innumerable requests !for information on hepatitis and cirrhosis of the liver. Years ago, all doctors knew well the epi demic type of jaundice which follow ed, usually, within 15 to 40 days after contact with jjj some otner Dr. Alvarez person who had infectious jaundice. The patients, practically all, soon got well, and so far as was known, there were no bad after-effects. Today, we know that this disease is due to a virus, often called virus A. Unfortunately, it hasn't as yet been grown outside the body, so that we don't know as much about it as we would like to. In the old days physicians were acquainted, also, with cirrhosis of the liver a hard ening and atrophy with shrinking-up of the liver that was seen in perhaps one in four alcoholics. In cases of marked cirrhosis, in the final stages, the patient's abdomen fills up, every so- often, with fluid which usually has to be removed through a hollow big needle. One of my jobs when I was an intern, over 50 years ago, was to "tap" the enor mous water-filled abdomens of old saloon-keepers who, too often, had sampled the liquor they sold. No History of Alcoholism Even in those days we knew that in a large percentage of the cases of cirrhosis of the liver, we could get no history of alcoholism, and hence often we had no idea why the per son had the disease. Then, during World War II per haps because ,of the frequent hypodermic injections of ..this and that, and particularly the widespread use of blood plas ma which did so much to save the lives of many of our baly-wounded soldiers there appeared a tremendous amount of what is now called serum hepatitis." Soon it was decided that there must be another virus, later called B which may or may not be re lated to virus A. It can pro duce jaundice usually after an incubation period of from 60 to 160 days. Now it appears that virus B can persist in the blood for as long as five years after the person has had an at tack of hepatitis. Worst of all, it is extremely refractory to destruction by heat, or by chemiral affpnt.s. Tnriav. wp that any instrumenti such as a hypodermic needle or an ear-puncturing lancet (for the taking of a little blood) cannot safely be steril ized in alcohol or carbolic acid but must be boiled in water for at least 30 minutes, or kept in hot air at 180 de grees centigrade for one hour. Every so often someone thinks he has solved the problem, only to find later that he hasn't quite done it. Every effort is being made to exclude as blood donors for blood banks and transfusions, the people who have had an attack of jaun dice; but this does not re move' the danger, and a blood transfusion is still something so potentially dangerous that I personally, would not accept one unless my life depended on quickly getting some more blood into my veins. Unfortunately, as yet, the experts are in the dark as to why only certain persons with hepatitis, or inflamation of the liver, go on to get marked destruction of the liver with a bad scarring or severe cirr hosis. That cirrhosis is not common after hepatitis can be seen from the fact that one group of investigators found cirrhosis in only two out of 114 persons who were studied from seven to 60 months after an episode of acute hepatitis. Another group of investigators did not find a single case of cirrhosis among 271 persons who, from four to eight years previously, had had an attack of hepatitis. Some men suspect that the person who develops a severe hepatitis with a subsequent cirrhosis is one who, when he got the virus infection, al ready had damaged his liver in some way, perhaps by the taking of alcohol or some other drug. Interestingly, the giving of alcohol in large amounts to experimental ani mals does not produce cir rhosis of the liver. Many men suspect that in the case of man, the drinker of alcohol sometimes gets cirrhosis be cause, while he is drinking, he does not eat much, and his poor nutrition helps to dam age his liver. No Satisfactory Treatment Unfortunately, as yet, there is no satisfactory treatment for either hepatitis or cirrho sis. Because hepatitis tends in all but a few cases to clear up by itself, it is hard to say whether a diet or medicine used turned the tide in the pa tient's favor. In large groups of patients, such as have been studied in the Army, the im pression has been that a full general diet, which nourishes the patient well, is better than any form of dietary re striction. Also, it appears that letting the patient get up and walk around is better than keeping him in bed. No drug seems to have done much good. The alcoholic who has a cirrhosis should, of course, quit drinking. If he goes on drinking, he is only asking for serious trouble and per haps death. Dr. Alvarez hopes his read ers will understand that it would be impossible for him to answer requests for infor mation or to attempt to diag nose by mail.
