Mayo Clinic Emeritus Professor of Medicine. Mayo Foundation L '1 A . I Ant 1 I Arthritis Hundreds of people want to know what they can do for arthritis. First, they should know what type of arth ritis they have. Some may have only some fibrositis, which is a mild sort of younger broth er of arthritis. It does not Dr Alvarez prod uce de formity of the joints. Spells of it tend to keep coming and go ing at intervals all through life. I have had it ever since I was seven years old, and still none of my joints are deformed in the slightest. This is in spite of the fact that, sometimes for months on end, I have been un able to close my hand. We do not know the causes of fibrositis and the two main types of arthritis. We do know something about those forms due to gout, allergy and nervous ness. One form is associated with psoriasis, but this does not help us much, because we do not know much about psoriasis. Most elderly persons get some arthritis which is often of the senescent variety. The bones get worn down and pounded over. Usually the worst injury is in j the hips and the knees.. This is particularly true if the person is very heavy. There isn't much one can do for senescent arthri tis except to give sedatives that will relieve the pain at night. Sometimes the injection of Hy drocortisone into the joint will work a miracle. Today, surgeons are putting in new hip joints made of a plastc or some metal, and this helps some people. Deformity of Joints The worst form of arthritis is the rheumatoid, in which there is often marked deformity of the joints, with great crippling and great pain. This is the one form of arthritis in which cortisone or one of the cortisone-like drugs help much. For those who want more information on the sub ject, I have a booklet (Wilcox, Follett, Chicago). One of the big secrets in using cortisone or one of the newer cortisones is to take each day only the smallest dose that will make life bear able. In all forms of arthritis, physi otherapy, with heat and massage and good care, can usually help a great deal. There is a disease which looks like arthritis, but is really due to nervousness. Gout generally comes in attacks of severe pain which commonly affect the big toe. The disease attacks men and women. Typical is the fact that usually between attacks, the man is comfortable.
