The Mental Health Campaign
During May devoted people
are going to go from door to
door asking for contributions to
the National
Association for
Mental
Health. This
is made up of
more than 500
state and local
mental health
a s s o c iations
throughout the
country. It is
Aivares a no n-profit
organization of laymen and it
has the approval of the National
Institute of Mental Health, and
The American Psychiatric As
sociation. Many of our mental hospitals
are a disgrace. The buildings
are ancient and they are hous
ing twice as many patients as
they were built for. In some
places the few doctors in the
institution have no time for
much treatment of the patients'
mental disturbances. With only
low salaries to offer, it is hard
for the superintendent to find
good attendants. No research
can be carried out.
The money that the Mental
Health Association is now asking
for will be used to help mental
patients before they go into a
hospital, while they are there,
and after they come out. It will
support reasearch projects; it
will establish clinics for the
thousands of disturbed persons
who cannot afford to go to a
psychiatrist; it will train per
sonnel for mental hospital, and
it will support the campaign to
get better mental hospitals built.
It will help in educating the
public in the prevention of men
tal illness.
Exploratory Operations
People ask me why they
should submit to an exploratory
operation. They say the very
term frightens them. Any good
doctor will admit that at times
such an operation is unwise
and perhaps avoidable. A better
diagnostic examination might
have shown that it wasn't neces
sary, or a wise old consultant
might quickly have made the
diangosis without it For in
stance, I have seen many a
woman whose abdomen was
opened because she had a pain
on the right side under her ribs.
Her gallbladder had been ex
amined by x-rays and found to
be functioning normally, but
still the woman's doctors were
uneasy about her. They operat
ed, thinking that they would
probably find some local disease
to account for her pain, but they
found none. If the woman had
known enough to emphasize the
fact that for years she had had
migrainous sick headaches, and
if the doctors had been taught in
college that women with a strong
tendency to migraine sometimes
have pain in the gallbladder
region, for which no local cause
can ever be found, the woman
would not have had to be operat
ed on.
Able professors of surgery are
now trying to teach their gradu
ate students not to cling to the
olj idea that when a patient has
some abdominal distress, an op
eration will usually find some
thing radically wrong that can
be removed. In hundreds of
thousands of cases, a discom
fort or pain in the abdomen is
due purely to some sort of a
nervous storm that is coming
down from the brain, or to the
eating of some food to which
the person is allergically sen
sitive, or to getting constipated,
or to getting a sick headache, or
to getting either arthritis in
the spine, or a related irritation
of the nerves leaving the spine,
or a fibrositis in the muscles
and so-called fascias (bands of
fibrous tissue) in the abdominal
wall.
Operation Justified
There is one type of case in
which I think an exploratory
operation is often justified and

even imperative. In such cases it
is not performed as often as it
should be. I am thinking of the
case of the man or woman past
midde age who.suddenly, out of
a clear sky, and for the first
time in his or her life, develops
alarming symptoms such as loss
of appetite, loss of weight, loss
of strength and the feeling of
well-being, or pains suggestive
of a stomach ulcer or of an in
testinal obstruction.
In such cases the physician
must immediately have the pa
tient examined with the greatest
care. He should have the blood
examined to see if there is any
sign of cancer growing any
where; he should have the di
gestive tract x-rayed from end
to end, and he should have the
rectum looked into with a light-
i ed tube. Finally, if nothing
shows up on examination, it
may be wiser to explore the
abdomen surgically than to wait
until even the neighbors can see
that the patient must have a
I cancer somewhere. Occasionally,
also, in the cases of young per
sons who pretty definitely have
some sort of severe illness in the
abdomen, it is better to operate
and look for it than to wait.
