The Medical Roundup lUfl. GLOSSITIS, OR ABNORMAL TONGUE Many ask me what to do for glossitis, or trouble with the tono;ie. and usually there is Jit- 'Wp1 tie I can Dre scribe. Fortu nately, in most cases, the pa tient does not need any treat ment. Perhaps he has a "geo- graphical tongue," which means that there is a vvaiiter Alvarez whitened area on it, perhaps with red margins that make one think of a map. That is why we speak of a geographical tongue. I don't know of any good treat ment for it. . Other persons are disturbed because the top of their tongue is deeply fissured. Again, I don't know if this is a disease, and. I don't know what to do for it. I suspect some persons were born with a tongue like that. Some persons complain of one or two little white sores which come and hurt a good deal for perhaps five days and then dis- canker sores. In my experience, they are generally due to the eating of some . food, such as chocolate, or tomatoes, or pine apple. If the patient will keep a record of the unusual foods eat en just before one' or two of these canker sores come, he may soon learn what he must avoid eating. Many persons worry a great deal about a coated tongue, but today I think few of us psysi cians pay much attention to it. There is good eyidence to indi cate that it is due to the regur gitation of gastric contents dur ing the night. Food comes up from the stomach through the gullet, and this is what coats the tongue. I don't know of any way of stopping the regurgita tion of the food. The Japanese make a little sort of hoe with which they .i it a . . , ,, uccin me tongue, aiso tney make their bamboo-h a n d 1 e d tooth brushes with a thinned segment which can be bent into a loop. With this, they scrape the top of the tongue. In Amer ica some persons take a wash-rag on the top of their finger and with this scrub the, top of the tongue. It is conceivable that this cleaning may lower the TV 1ST? Missing Beats of the Heart Persons ask me about so-called missing beats of the heart. For tunately, in nearly all cases these beats do not constitute a serious disease. They are compatible with a long and comfortable life. Usually, in the cases, the heart, by all tests, is perfectly normal. The extra beats seem to be sent in by way of the nervous system. What happens is that when a tiny beat comes, the next beat is blocked and hence . omitted. With this, the heart gets a rest, and so the next beat is a bfg one. This is the beat that the person feels as a thump, or a "turning-over of the heart." For years I kept in my office a big book on heart diseases writtei.'by the world's greatest authority on the subject Sir James Mackenzie, of London. I had a mark to help me open to the page on which Sir James trasys'toles. All he said was, "The trasystoles. Al Ihe said was, "The patient must learn to get used to them" Sometimes if the extra beats come very frequently and are very disturbing, the doctor will give a drug called quinidine. This can quiet the heart muscle. Glaucoma One of the commonest causes of blindness in adults is glau coma. Many persons ask me about it and how it can be re lieved or cured. I think usually if a man once develops glau coma he has to fight it or at least watch it for the rest of his days. In this disease the tension of fluids in the eye-ball becomes top great; more fluid is formed inside the eye-ball than can ooze out during an interval of time. This increased pressure can in terfere with the circulation of the retina (or seeing part of the inside of the eye) and this can cause serious trouble. In most cases I think the patient gets by with drops, usually of a drug called pilocarpine; I have known many patients who kept putting in these drops several times a day and every day. Others have a little operation done in which a hole is drilled with the idea of letting some of the, fluid put. Recently, a drug called Dia mox nas been used; I understand it. helps fluid to get put- of the eye. ;i',-?r.y J ..
