Inso far as abdom inal pain is concerned, its character is too variable to be regarded as an Infallible guide. Abdoml n a 1 pain in chil dren often ac companies the early develop ment ot con t a g 1 o u s dis eases. C h 1 1- Dr. Brad dren may als'j react to certain dietary excesses or infections much mora violent ly than adults. The doctor who examines the child complaining of abdominal pain must consider a greai many factors. No doctor wants to in flict a useless operation upon anybody, but the "masked" form in which appendicitis sometimes annears. leaves him no choice but to proceed with the greatest caution. Perhaos the most cautious course is to operate whenever the presence of appendicitis can not be disproved. Actually, ie conservative course is to oper ate. The doctor who withholds operation pending developments assumes much more responsibil ity than the doctor who decides to operate on suspicion. If a doctor operates for sus nected aoDendicitis and finds a normal appendix wnicn some times haDDens even to doctor's children, no great harm is done. The patient loses a useless ap- penaage wnicn n ivn in 6iii be the cause of death at some later day. The operation itself, carries a very small risk and disability is very limited. If a normal appendix is louna : at operation, it is always remov ed. It is such a useless organ that there is not the slightest reason for letting it remain. As a mat ter of fact, it is a generally ac cepted practice to remove the ap pendix whenever it can be con veniently managed, when the ab domen is opened for some other main surgical purpose. Despite all this, no one is oper ated upon, even In doubtful cases, until the doctor satisfies himself that surgery is the saf est course. Occasionally, appen dicitis may be so apparent that nothing more than a perfunctory examination is needed to make the diagnosis. Nevertheless, once the patient is in the hospital fur ther tests and examinations are made to substantiate the diag nosis. The development of the "mir acle drugs" has not lessened the necessity for expert surgical judgment and management. The treatment for acute appendicitis IN CHILDREN is surgical removal not the ad ministration of "miracle drugs." The inability of a frightened and pained child to cooperate with a doctor and to respond to his questions makes the doctor's task more difficult. The frequent appearance of abdominal pain in children as the result or accom paniment of other disorders not related to appendicitis, adds to the difficulty. It requires a high degree of skill and competence to distinguish one from the oth er. Again I repeat that if a mis take is made and a normal ap pendix is found at operation, it may prove to be a blessing In disguise. So unreliable is abdominal pain as an " indicator" of appen dicitis, tnat otten a ruptured ap dendix is found in cases in which pain is extremely slight. In other cases, extreme pain may exist in the presence of a very slight appendicitis or no appen dicitis. Fortunately, tne doctor is not guided by or limited by the factor of pain alone. The history of the case, the pulse, the temperature, the fac ial expression and position of the child together with the "feel" of the belly, enables the doctor to make his provisional or preliminary diagnosis. Later, laboratory work is used to sub stantiate or to undermine the "working" diagnosis. Finally, never, never, give a child a laxative, who is com plaining of abdominal pain.
