SINUS DISEASE REAL AND IMAGINARY "I've got sinus," is an expres sion often heard. The expression is eramalirallv atrocious and medically meaningless. A sinus is a space or c a v 1 1 y, of which the body has several of different varie ties. "Air sinuses," known medi cally as Para na s al sinuses, are hollow spaces adjacent to the nose and contained Or. Brad within the bones of the face and head. These spaces communicate with the interior of the nose through small openings. They are lined with a membrane which is continuous with the membranous lining of the Inte rior of the nose. They may be considered as alcoves leading out of a large room, Many years ago and occa sionally even now, the word "ca tarrh" was often used, Persons who suCfcrcd a persistent dis charge or over-secretion from the nose were thought to be suf fering from catarrh Catarrh was blamed for variety of ailments not only of the nose, ears and respiratory apparatus in general but also for afflictions of the stomach, bowel and urinary ap paratus. Certain germs were uniformly i frtii,fl in ".oinrrhol" rtiwnharOnc and for a time were thought to be the cause of the disorder. The passage of time witnessed the disappearance of this notion, along with advancing knowl edge of disease in general. Rather suddenly, the word ca tarrh gave way to "sinus dis ease." Certain segments of the public soon reduced this expres sion to "sinus." The notion that a persistent discharge from the nose was due to sinus infec tion very often it ia soon be came firmly established. The air sinuses of the head are often infected. The Infection may be acute and subside within a few days, or it may linger on and become chronic. It is highly probable that all of the nasal sinuses are infected to some ex tent, each time a person acquires a "cold," or other infection of the interior of the nose. An acute Infection of one or more sinuses may give rise to considerable pain in the sinus area, along with the usual symp toms of stuffiness and discharge. Pain developing below the eye or in the forehead sometimes in other areas of the head, Is in dicative of sinus infection with obstruction of the sinus opening. Chronic sinus infection is in variably associated with obstruc tion at some point which inter feres with drainage. This ob struction may be due to bony ir regularity or to the presence of abnormal soft tissues. Many of these obstructive dis orders can be permanently re lieved only by expert surgery. Some soft tissue obstructions can be removed by electro-surgery and occasionally by the injection of solutions into the obstructive tissues which results in later shrinkage. Persistent discharge from the nose is not always due to sinus disease or "sinus." Abnormal sensitivity of the local tissues allergy accounts for the diffi culty in many Instances. A persistent, offensive dis charge from the nose, accom panied by shrinkage of the nasal membrane and crusting, Is known ai "ozena." This disorder does not seem to be related to sinus disease as such and is very resistant to treatment aimed either at the sinuses or the im mediate condition itself. Frequent passing from warm to environments causes a na tural and healthful, increase in nasal secretions. This condition or effect Is in no way to be con sidered as "disease." Persistent discharge from one side of the nose In children is often found to be due to some foreign body In the nose, such as a bean or button. Sinus disease Is admittedly a major factor In the cause of per sistent nasal discharge. Admit tedly, it is not the only one. 5. f.f" (l KENflLE. In fact every infant or child with a profuse slightly bloody nasal dis charge persisting longer than a few days and producing erx coriatton of the lip, should be suspected of having nasal diph theria, and a culture should be taken to determine whether the trouble is diphtheria.
