Calif. . , IT'S AN OLD BP If this premise that obstruc tion of breathing and Impair ment of hearing are the only good reasons for subjecting the child with enlarged ton- sils a n d o a d e n o ids to surgical treatr ment is cor rect, as I con tend, the medi cal profession is still preuy much at sea in regard to the Dr. Brady question ol en larged tonsils. Aside from tacit acceptance of the idea that lymphoid tissue, which composes the fauclal ton sils, the uharyngeal tonsu taae- noid body) the lingual tonsil (masses on the root of the tongue), the lymph nodes in the necli (called "kernels wnen en larged), the lymph nodes around the roots of the lungs ana De- hind the deep bronchial tubes, the mesenteric lymph nodes (in the abdomen), the ring of lym phoid tissue around the base of the appendix, etc., eve., wnere ever situated, lymphoid tissue is directly concerned with the de fense of the body against Infec tion, particularly in childhood, aside from admitting that this may be the function or purpose of the tonsils and adenoids in growing children, the rank and file of the medical profession 30 years ago regarded and some practitioners today regard en larged tonsils as pathological or abnormal and on this arbitrary and unsubstantial theory they advise or practice the atrocity called "tonsillectomy" which serious major operation, would remind the world and the backward portion of the medl cat profession, seldom "gets" quite all of the tonsil. If any self-constituted spokes man on the ground and lofty surgical performers of the nine teenth century school feels Im pelled to challenge my state ment. I'll be obliged to repeat some statistics and reports by qualified pathologists who ex amined what some representa tive operators removed from the throat in routine tonsillectomies. Twenty years ago Dr. R. B. Canfield pointed out (Ann. Clin ical' Mad. '26 4:1058) that chil dren deprived of their tonsils be fore the age of five years are more susceptible to the acute in fectious diseases than are chil dren whose tonsils are not re moved. Fifteen years ago Dr. Albert D. Kaiser concluded from his studies on thousands of school children in Rochester, N. Y., (Children's Tonsils In and Out, Llppincott Co., '32), and twenty years ago Dr. J. A. Doull (Publie Health Reports, Aug. '24) concluded from similar studies among Baltimore children, that children whose tonsils have been removed are just about as Sus ceptible, or as immune, as you prefer, to scarlet fever as are children whose tonsils have not been removed. i rom these and many similar observations by physicians who still seem to approve the more or less indiscriminate removal of tonsils, I infer that, aside from obstruction of breathing and im pairment of hearing, either of which warrants surgical, dia thermy or X-ray treatment of enlarged tonsils andor adenoids, the old timers still in practice continue perpetrating the tonsil lectomy atrocity on children mainly because they don't know any better.
