2A COTTAGE GROVE SENTINEL MARCH 1, 2017 BIRTHS Jeremiah Matthews 1995-2017 Jeremiah J. Mathews, 22, of Cottage Grove died May 6, 2017 from com- plications from a car accident that happened May 15, 2016. He was born March 19, 1995 in Eugene to Randy and Shawnee (Lane) Mathews. He graduated high school and enjoyed hunting, fi shing, camping and 4x4 wheeling. Survivors include his partner, Brandi Ewing, daughter, Haylie of Cottage Grove, mother, Shawnee Mathews, Joe and Leatha Lane of Las Vegas, Nev., and Donald and Jane Mathews of Cottage Grove, fi ve sisters, Sarah Tierce of Albany, Ore., Amber Mathews of Cottage Grove, Ashley Mathews of Greenville, S.C., Amanda Patton of Cottage Grove and Candice Parsons of Cottage Grove, and many aunts and uncles. He was preceded in death by his father, Randy Mathews. A memorial was held May 13 at 2 p.m. at the Seventh-day Adventist Church, 820 S. 10th St., in Cottage Grove. Arrangements under the care of Andreason’s, Springfi eld. Barbara Kofoid 1931-2017 Barbara L. Kofoid, 85, of Cottage Grove died May 8, 2017. She was born Sept. 20, 1931 to Leon and Irene (Mitts) Godard. Barbara was just fi ve years old when her mother passed away due to complications of childbirth. She was raised by Ruby (Mor- ris) Godard. Barbara graduated from Cottage Grove High School and later received her nursing degree from Lane Community College at the age of 52. She married Loran Levings in September 1949 and they later divorced. She married Gary Russell Kofoid in August 1986. She worked as a Registered Nurse and taught preschool and kindergarten. Her greatest love was being a homemaker. Barbara’s hobbies and interests included gar- dening, sewing, tole painting, camping and loving her grandchildren. She belonged to Our Lady of Perpetual Help Catholic Church, Delight Valley Church of God, and Gardening Club. Barbara is survived by her husband, Gary Ko- foid of Cottage Grove, four daughters, Susan Kruse (David) of Shedd, Ore., Debra Levings (Ron Bryan) of Looking Glass, Ore., Lori Eck- stine (Jerry) of Cottage Grove, Patti Eckstine (David) of Creswell, Ore., a step son, Kevin Ko- foid of Cottage Grove, a brother, Harold Godard of Roseburg, Ore., two sisters, Terry Holden of Phoenix, Ore. and Pat Hackett of Ashland, Ore., 17 grandchildren and 53 great-grandchildren. A celebration of life will be held at 2 p.m. on Thursday, May 18, at Smith-Lund-Mills Funeral Chapel in Cottage Grove. A reception following the service will be held at OLPH Catholic Church, 1025 N. 19th St., Cottage Grove. Memorial con- tributions may be made to the Cottage Grove Genealogy Society. Arrangements under the care of Smith-Lund-Mills Funeral Chapel, Cottage Grove. Bernice M. Green Raymond 1922 – 2017 Bernice M. Green Raymond, age 94, passed away peacefully on May 4, 2017. She was born in Utida, Utah on September 25, 1922 to John Henry Ar- myon and Mary Anderson Armyon. She lived in several small communities in Southeastern Idaho as a child. Th e family then moved to Lava Hot Springs, Idaho, where Bernice attended high school. She was on the women’s basket- ball team and worked on the school newspaper. Aft er graduating, she moved to Salt Lake City, Utah, where she worked at an arms plant in- specting ammunition during World War II. In 1943 she married Kenneth Jay Green. Aft er he was honorably discharged from the Army Air Corp, they moved to Idaho Falls where their daughter Dianne was born. Th ey moved to Oregon in 1947, and that year, their son Stephen was born. Th ey moved to Cottage Grove in 1948, where Bernice lived the remainder of her life. In 1949, Kenneth died as a result of his service in the Army Air Corp during World War II. In 1953, Bernice married Gordon William Raymond. A year later, their daughter Debra was born. Bernice lived a life of devotion to her family. She had many talents, including crocheting, sew- ing, all kinds of craft s (which she willingly taught to others), and she was an incredible cook! She never had a piano lesson, and couldn’t read music, but played the piano beautifully. Bernice loved the beach and spent many vacations there with her family. She also loved watching the Portland Trail Blazers games, Oregon Ducks and BYU games. Bernice was actively involved in her church until health issues prevented it. She held leader- ship and teaching positions in the children’s organization, the youth organization and the women’s organization. In 2008, because of health issues, she moved to Magnolia Gardens Assisted Living Center where she made many new friends. Bernice was preceded in death by her parents; her brother Harold; her sisters Helen and Lor- raine; and her husbands Kenneth and Gordon. She is survived by her sister Joan; her children; Dianne, Stephen and Debra; 8 grandchildren, and 23 great-grandchildren. Bernice’s family would like to express their appreciation to the staff and caregivers at Magnolia Gardens Assisted Living, for the kind and loving care of their Mother for the past nine years, and to Signature Hospice for their special care during her fi nal days. A service will be held on Saturday, May 20th, at 1:00 p.m., at the LDS Church locat- ed at 531 S. 10 th St., in Cottage Grove. Parking will be on the north and west sides of the building (enter through the west door). He was born Nov. 22, 1950 in Cottage Grove to Lucille and Paul Cook. He married Judith Cook. They later divorced. Doodles worked as a roofer and logger. He is survived by his life partner, Karen Norris of Creswell, mother, Lucy Cook Norris of Creswell, four children, Dina Cook of Cottage Grove, Wen- dy Cook of Springfi eld, Misty Hammett and Rusty Butler, both of Siletz, three brothers, Roger Cook of Texas, Doug Cook and Jim Cook, both of Creswell, two sisters, Becky Work of Cottage Grove and Melody Delorme of Eugene, ex-wife, Judith Cook of Eugene, and ex-partner, Lorraine Butler of Siletz.
