Our message isn’t always received well (or at all), since a compet- ing message is more ap- pealing…the benefits of survival don’t outweigh suffering the harms (“pos- sible” incontinence or im- potence). We’ve got you COVERED As we acknowledge, ev- ery man must make his own decision whether to be screened. What pains us is that while some men are fore- going screening, we are re- membering the 40-50 men who are no longer living or attending our meet- ings because they were screened due to symp- toms (probably already metastatic), were screened prior to symptoms (but already out of the pros- tate) or a victim of a very aggressive cancer. These men became our friends as we supported them on their journey, but more than that, they were hus- bands, fathers, grandfa- thers, uncles, etc. There is no replacing what their families lost. These men give us the drive to “brazenly,” if you will, confront men with the question, “Do you know your PSA number?” Too often, we get this re- sponse: “What is that?” Often, that leads to fur- ther discussion about the screening tests, when they should be done and risk factors (affecting when screening should start). Speaking of risk fac- tors, ever heard the word “genetics?” Let me bring you up-to-date with some information from the Prostate Cancer Founda- tion (PCF), the world’s leading philanthropic or- ganization dedicated to funding life-saving cancer research. The most famous that you may have heard of are the BRCA1 and BRCA2 mutations that increase risk for breast and ovarian cancer. In a 2016, PCF-sup- ported study of men with metastatic prostate can- cer, more than 10 percent had inherited cancer risk genes such as BRCA1, BRCA2 and at least 18 other newly-discovered genes that may be im- portant to risk of prostate cancer and other types of cancer. If a man discovers that he is a carrier of an inherited mutation in the BRCA1, BRCA2, or other genes that increase risk for prostate cancer, this has critical implications for all his family members. If male family members have inherited the same mutation, they may be at increased risk for prostate cancer and other cancers such as male breast and pancreatic cancer. And if female family members have inherited these mutations, they may be at increased risk for pancreatic, breast, ovari- an, and endometrial can- cer. These mutations may also increase risk for oth- er cancers, depending on the gene. Therefore, it is important to look at can- cer risk across your whole family. Inherited gene mu- tations can cross many forms of cancer, including but not limited to, pros- tate, breast, colon, ovar- ian and pancreatic. It is very important to learn and share with your doc- tor what cancers have oc- curred in other members of your family. More than 20 different genes have been identified that run in families with prostate cancer (heredity prostate cancer). Family members who learn that they are carriers of these mutations need to discuss their findings with genetic counselors and their doctors to bet- ter understand their can- cer risk, options for early detection, and how to re- duce risk for various other forms of cancer. Next month, we start a prostate cancer journey that brings a local family face to face with the above. Dad is diagnosed, treated for an aggressive prostate cancer and undergoes ge- netic testing. Two daughters and a son are impacted as dad shares the results of the testing. Follow along as his daughters and son deter- mine what that means for each of them.
