SIUSLAW NEWS ❚ SATURDAY, FEBRUARY 17, 2018 Port to hold meeting, work session Feb. 21 On Wednesday, Feb. 21, the Port of Siuslaw Board of Commissioners will hold a work session at 6 p.m. and their regular meeting at 7 p.m. at the Port conference room, located at 100 Harbor St. in Florence. The work session will include a presentation by Dave Harlen with Business Oregon. The regular meeting will include an update on the Erosion Repair Project and a resolution for an appropria- tions transfer. Wine tast- ing, food and chocolate were enjoyed by all in attendance. This is an annual fundraiser for the organization and the event was sold out. Over $800 was raised and tickets were limited to 40 people. Peace Harbor Hospital cafeteria staff provided most of the food and desserts. Bridgeport Market owner Jay Cable and the staff provided and served wine and cheese. The event was organized by Joan Kerrebrock and Gail Petty, both past presidents of the volunteers. Due to its success, there are plans to hold this event again next February. COM New pilot program taps paramedic for home visits PeaceHealth Peace Harbor Medical Center and Western Lane Ambulance District have partnered to pilot a new pro- gram aimed at preventing hos- pital readmissions by enhanc- ing in-home care. Launched Jan. 2, the Mobile Integrated Healthcare program provides non-emergency EMS visits to patients in their homes following hospitalization or an Emergency Department visit. A 911 call to EMS frequently results in transport to an emer- gency room, even when the patient may not require that complex and costly a level of care — and if EMS does not transport the patient, they are not reimbursed for the expense of ambulance activation. Working with Peace Harbor community health worker Heather Hammond, paramedic Chris Martin meets daily with discharge planners and makes every effort to visit the patients while they’re still in the hospi- tal to explain how he can help with their post-discharge care. In addition to providing fol- low-up evaluations, Martin also serves as a connection with other health and social services, making sure patients understand how to access food and transportation resources and other community services. In rural areas like Western Lane County, community para- medics can help fill gaps in local healthcare delivery relat- ed to primary care provider shortages and long travel times to the nearest hospital or clinic. An experienced program man- ager, Martin is gathering met- rics to measure the success of each of the pilot program’s four goals: • Reducing hospital readmis- sion rates • Reducing ED visits • Reducing non-transports (when an ambulance is dis- patched but the patient is not brought to the hospital) • Reducing the percentage of patients seen at Peace Harbor for discharge follow-up The program hit the ground running in its first week, when Martin provided immediate post-discharge visits and triage to 12 patients who had been referred to Home Health Care. Kathy Murphy, RN, CHPN, manager of Home Care Services, is pleased that the program will help her staff identify patients who would benefit from home care sooner than might otherwise occur.
