IN THE TRIBAL COURT OF THE CONFEDERATED TRIBES OF THE GRAND RONDE COMMUNITY OF OREGON CASE NO. C-12-04-001 DECREE CHANGING NAME In the Matter of: PETITE, Kristi, Petitioner, For HALL, Jesse Izayah, a Minor Dased on the allegations set forth in the Petition for Minor Name Change filed on April 2, 2012, and the sworn testimony of the Peti tioner at the Show Cause Hearing held on May 30, 2012; THE COURT FINDS: 1. Petitioner is over 18 years of age and is the parent or legal guardian of Applicant, a minor child; 2. The minor child is an enrolled member of the Confederated Tribes of the Grand Ronde Community of Oregon; 3. The minor child has not been granted a change of name by the Tribal Court within the past twelve (12) months; 4. The minor child has been known by the names of Jesse Izayah Hall and Izayah Jesse Hall. The child and both of his parents wish to formally change the child's legal name to Izayah Jesse Hall. 5. No objection to the Petition has been filed by any parent or legal guardian of the minor child and no objection was raised at the Show Cause Hearing; and 6. The proposed name change is in the best interests of the minor child. THE COURT DECREES: 1. That the name of the minor child be changed from Jesse Izayah Hall to Izayah Jesse Hall; 2. That the Clerk of the Tribal Court shall cause a copy of this Order to be published in the Tribal Newsletter; 3. That the Clerk of the Tribal Court shall submit a certified copy of this Order to the Tribal Enrollment office; 4. That the Clerk of the Court shall provide a certified copy of this Order to be submitted to the Registrar of Vital Statistics in the state in which the applicant was born; 5. That the Petitioner, on behalf of the minor child, shall surrender all old Tribal identification cards bearing applicant's former name to the Tribal Enrollment Office prior to the enrollment office issuing applicant new Tribal identification cards bearing the applicant's new name; 6. That within sixty (60) days from the date of this order the Tribal Enrollment staff and if necessary the Tribal Enrollment Committee shall cause new Tribal Identification cards to be issued to Petitioner showing the new name of the Tribal member and shall change the of ficial membership roll and all official enrollment records of the Tribe to reflect the Petitioner's new name as stated in this Decree. Tim loved fishing. He made flies and lures and sold them all over the world. He was river guide in Oregon and also was a marathon runner, race car driver and carpenter. He is survived by his wife, Ba bette; sons, Justin, Austin and Ian; parents, Marvin Warren and Evelyn and Leo Seidel; five sisters, two brothers and 23 nieces and nephews.
