Alternates are appointed by Tribal Council and serve in lieu of board members who are disqualified from serving or unable to serve in an election. Alternates may also observe election functions and perform incidental tasks at the request of the board. If you are interested in being an alternate and you or an immediate family member are not planning on being a candidate in the 2006 Tribal Council Election, please send a letter of interest to Claudia Leno, Election Board Chair, P.O. Tribal Members Requested to Participate in Online Survey The Confederated Tribes of Grand Ronde (the "Tribe") strives to continue providing quality health care for Tribal members, with a focus on access of care, quality of care and cost of pro viding care. Your participation is requested to collect information for the Tribal Member Health Plan as currently administered by United Medical Resources (UMR). The purpose of the survey is to gather information about Tribal members, and their access to other health insurance coverage through alternative carri ers (via your employer, Medicare, etc). Tribal members eligible for primary coverage through othej carriers are required to provide information about the altjer-' native carriers. Providing this information will not change health benefits to Tribal members, but will assist the Tribe in continuing to provide access to quality health care for all Tribal members while helping to manage cost of care. Responses to this survey will be strictly confidential and will be available only for the above mentioned purpose. The survey will be available online from January 23, 2006 through March 10, 2006. Members will also receive the paper survey via first class-mail the week of January 16, 2005, however, online participation is encouraged to (1) enter you promptly into the drawing, (2) reduce the chance that your responses may be lost in the mail, and (3) save on the cost and time of mailing. Al though no action is requested at this time, the direct web ad dress for the survey will be posted in future communications. Members will also have access to the survey through the Tribe's website at www.grandronde.org. Information to be posted at the Tribe's website is forthcoming. You're input is crucial and so is a successful return rate; there fore, we are organizing a drawing with great prizes in return for your participation. In addition, there will be an extra in centive for those who 1.) Complete the online survey by March 10 or 2.) Who return the paper survey by March 6. LOW-INCOME ENERGY ASSISTANCE PROGRAM The Low-Income Energy Assistance Program will begin scheduling appointments December 1, 2005 to assist households with their heating bills. Appointments will begin January 3, 2006. Residents of Marion and Polk counties whose household income is within 60 median income (Oregon statewide average) guidelines may apply. All appointments are first come, first serve. Applicants will need to bring in proof of income for all persons age 1 8 and over, birth dates and social security numbers for all household members, proof of subsidized housing, and a current heating bill. Income includes gross wages, self employment, unemployment compensation, public assistance, social security benefits, Veterans benefits, work-study, child support, interest, etc. Deductions will not be allowed.
