Casino Revenue Distribution 2003
The Tribal Council has passed a resolution authorizing a distribution of
Casino revenue to Tribal members through the Member Benefits Depart
ment. D Distribution Date: December 11, 2003
B Eligibility: In order to receive a per-capita payment, you must be
living as of January 1, 2003, and an enrolled member of the Tribe as of
December 5, 2003. A check will be automatically generated for members
18 years of age as of December 11, 2003. Children born in 2003, assum
ing they meet all other eligibility requirements, are eligible for the Decem
ber 11, 2003 distribution. For those members under the age of 18 funds
will be distributed into their trust accounts. The amount is not yet
determined and we will not have that information until sometime
the first week of December.

address or have other permanent changes please contact: Mem
ber Benefits Department 1-800-422-0232:. Liz Leno extension 2082,

cannot distribute a check without a Social Security number and
this includes minors' distribution. THE DEADLINE FOR AD
DRESS, OR ANY OTHER CHANGES WAS NOVEMBER 30, 2003.
D Checks may be picked up at the Tribal Gymnasium, located at
the Education Facility on December 11, 2003, between the hours
of 8:00 a.m. 4:30 p.m. No checks will be picked up before 8:00am.
Any checks not picked up that day will be mailed to the address
we have on file on December 11, 2003.
Q Payments from Casino revenues are subject to federal and
where applicable, state income taxes. Tribal members are respon
sible for any tax due as a result of this payment.

RE-ENROLLMENT IN THE ODS
MEDICAL AND DENTAL PLANS FOR
THE 2004 OPEN ENROLLMENT PERIOD
In an effort to update our current insurance information, we are asking
each Tribal member to re-enroll in the ODS Health and Dental Plans.
Even if you are currently enrolled, we are requesting that you still fill out
an application and re-enroll.
The purpose of re-enrolling each member is to provide the Tribe with
updated member information, including whether or not you have other
insurance available to you (i.e. Oregon Health Plan, Medicaid, Medicare,
another insurance company, etc). If you do not re-enroll, you will not be
taken off the insurance programs. We are just requesting your assistance
in this endeavor and appreciate your help.
Each Tribal member has been mailed a letter with a new application
and return envelope. Please make every effort to return your Medi
cal and Dental insurance applications by Friday, December 19,
2003, to: The Confederated Tribes of Grand Ronde, Attention: Member

Shop With The Sheiriff December 13
This year the "Shop with the Sheriff program will be held on Sat
urday, December 13. Members of the Tribal Council have once again
volunteered to drive between 20 30 Grand Ronde Elementary school
age children to the Wal-Mart store in Dallas to participate in a Christ
mas shopping spree.
The local Grand Ronde Service Integration Team, working with
the Willamina Elementary School staff will select the children to par
ticipate in the program. SIT members will meet with the Deputies in
Dallas and help escort the children through the store. The children
are encouraged to purchase presents for their family members.
The Dallas Wal-Mart store has financially assisted the "Shop With
The Sheriff program since its start. Wal-Mart employees further
assist the children by providing the young shoppers with small gifts
and providing an opportunity to visit with "Santa."
Afterward the children are brought back to Grand Ronde where
SIT members help the children wrap the gifts before taking them
home.
