Smoke Signals FEBRUARY 1, 2001 Gift Left at Cemetery Cemetery Groundskeeper and Tribal Elder Russ Leno was sur prised recently when someone left him a gift as a way of saying thank you for the work he does at the cem etery. The gift, a very nice pocket watch and chain, was left for Leno by Connie Brown. Brown said she wanted to do something special for Leno and left the gift when she kept missing him at the site. (SMGI) Y L A Contact Mike Larsen at 503-879-2407. 1 : 1 Grand Ronde Health & Wellness Center The Grand Ronde Health Center is experiencing a high rate of patients losing or having their prescription medications stolen. This results in thousands of dollars in replacement costs that could otherwise be used for other patient medical care services. To help reduce lost or stolen prescription medications, the Health Center will implement the following policy guidelines. MAINTENANCE MEDICATIONS Maintenance medications, other than controlled substances, which have been reported as LOST can be refilled only as follows: 1. In instances when maintenance medications have been reported as lost, it will be noted in the patient's record along with details provided by the patient. 2. A replacement supply (sufficient to provide medication up to the next autho rized refill date) may be dispensed at the discretion of the pharmacist. That decision will be based upon the patient's condition and the medication involved. The pharmacist shall report the event to the prescribing i practitioner or medical director within 24 hours. 3. If maintenance medications are reported as lost more than once within a year, the patient will be required to see the medical provider before the medication is dispensed. In addition, a patient will be limited to a two week supply with a dosage regimen provided by the pharmacist. The patient may also be referred for counseling andor assistance in managing prescription medications. 4. Patients with Indian Health Service (IHS) eligibility will not be charged for the lost medication. Patients without IHS eligibility will be responsible for the partial fill fees and charges for the medication. Maintenance medications, other than controlled substances, which have been reported as STOLEN can be refilled only as follows: 1. In instances where maintenance medications have been reported as stolen, the patient is expected to complete a police report and submit the report to authorities. (A copy will be placed in the patient's medical record). 2. A replacement supply (sufficient to provide medication up to the next autho . rized refill date) may be dispensed at the discretion of the pharmacist. . The decision will depend on the patient's condition and medication that is involved. The pharmacist will report the event to the prescribing practitioner or medical director within 24 hours. 3. If maintenance medications are reported as stolen more than once a year, the patient will be required to see the medical practitioner before medication is dispensed. The patient will need to wait until the next regular date to refill a prescription for lost or stolen controlled sub stances medications.
