12 Smoke Signals HIGHER EDUCATION & TRAINING HIGHER EDUCATION PROGRAM A federal and Tribally funded "heed-based" program. Requires full-time attendance. Requires that students file for Federal Financial Aid each year. The application takes between 45 and 60 days to process. ADULT VOCATIONAL TRAINING PROGRAM A federal and Tribal funded "need-based" program. Requires full-time attendance. Requires that students file for Federal Financial Aid each year. The application takes between 45 and 60 days to process. Deadline dates to be considered for funding in Higher Education or AVT Program are: July 1: Fall TermSemester November 1: Winter TermSpring Semester February 1: Spring Term Summer Term funding is available, but students must meet eligibility criteria. EDUCATION TRUST FUND SCHOLARSHIPS The scholarships are renewable as long as the student meets minimum academic require ments. Scholarship applications are mailed out as requested from January until dead line date of April 30 of each year. The completed application must be received or postmarked by April 30. The following scholarships are offered: Graduate full-time and part-time in the amounts of $10,000$5,000. Undergraduate full-time and part-time in the amounts of $6,000$4,500$3,000. Vocational Training full-time and part-time in the amounts of $6,000$3,000. Eula Petite Memorial Scholarship in the amount of $7,000. These are competitive scholarships. Each scholarship application is reviewed and ranked on a points basis. Applicants are rated on academic merit, leadership and Tribal and community involvement. It is required that applicants submit an essay with their application along with other documentation. SHORT-TERM TRAININGCONTINUING EDUCATION PROGRAM Provides educational opportunities to individuals who take classes or training at ac credited colleges, universities, private training institutes, apprenticeship, certification or licensing programs. Limited assistance for tuition, books and fees for students attending school or training on a part-time basis. Students can access up to $800 per termquarter. Students need to complete a program application at least two weeks prior to the first day of classtraining. Verification of registration must be sent to the Tribal Education office each time a student registers for a new term, training, or class. Funding assistance is authorized on a per term, per class system. Examples of classes and trainings are: Chemeketa classes, CTV (teleconference) classes, job enhancement classes, degree program classes, First AidCPR. ADULT EDUCATION PROGRAM Provides on-site individual and classroom instruction. Provides funding for GED testing fees. GEO pre-testing and assessments can be scheduled on-site. GED program can be accessed on the computers in the on-site computer lab. Specialized community education classes can be coordinated. Study materials available. JANUARY 1-30: Make copies of your completed Federal Financial Aid application. Mail the original in the envelope provided; send a copy to the Tribal Education Dept.; and keep one for yourself. JANUARY 1-APRIL 30: Request and complete the Tribal Education Trust Fund Scholarship program application. Mail to the Tribal Education Department. BEGINNING OF EACH TERMSEMESTER: Mail new class schedules to the Tribal Education Dept. END OF EACH TERMSEMESTER: Mail grade reports to the Tribal Education Dept. MONTHLY: For students attending a proprietary school, you need to send a grade report, progress report, and attendance record every month to the Tribal Education Dept. THROUGHOUT THE YEAR: Maintain contact with the Tribal Education Specialist. STUDENT RESPONSIBILITIES 1AU students are required to file for Federal Financial Aid. Continuing students are expected to file prior to February 15 of each year. A copy of the Federal Financial Aid application must be submitted to the Tribal Education Program in order to determine program eligibility. 2 All students are responsible for having their grade reports sent to the Tribal Education Program upon completion of each TermSemester. Either make ar rangements with the Registrar's office to have the reports sent directly to the Tribal office or mail a copy of your grade report to the Tribal office yourself. 3 All students must maintain a 2.00 GPA and successfully complete a minimum of 12 credit hours per termsemester to remain eligible for funding. 4 Students are allowed one term of Probationary Status. Students who are placed on PROBATIONARY STATUS will not be awarded the following term until they have provided the Tribal Education Program Specialist a copy of their grade report transcript. Transcripts must include the term of probation and must show success ful completion of 12 credit hours and verify that a 2.00 GPA has been earned. NO AWARD WILL BE MADE UNTIL TRANSCRIPTS HAVE BEEN SUBMITTED. 5 It is the student's responsibility to remain in "good academic standing." If a student demonstrates an on again, off again pattern of being placed on Proba tionary Status, they will be placed on probationary status for one complete aca demic year. Failure to meet minimum academic and or attendance requirements during the probationary termsemester will result in suspension from the program. 6 Students, please notify the Education Department of address changes and tele phone number changes as soon as they occur. 7 If emergencies occur that effect your attendance notify the Education Program and the college IMMEDIATELY! (Tribal Education Specialist, College Registrar's Office, College Financial Aid Office.) 8 Students who withdrew from school due to medical or health related reasons must obtain a Statement of Verification from their physician or health care provider identifying need to withdraw from schooL Students must also obtain a State ment of Releases from their physician that identifies ability to return to schooL Copies of physician's statements must be submitted to the Education Program Specialist. 9 All students must sign a "Release of Information" form regarding attendance, financial aid transcripts, education budgets, progress and grade reports. A Students, be aware that if your quit school or withdraw early from school X w you will be required to reimburse the Education Department monies you re ceived to attend school. n Students entering their SENIOR year are required to have a Graduation Au ditEvaluation completed Fall TermSemester and provide a copy of the AuditEvaluation to the Tribal Education Program Specialist. ACHIEVEMENT & RECOGNITION Applications are mailed out by March 15. Applications must be post marked by May 1, no exceptions. Eligible: CTGR Tribal member grades K-12. SUMMER SPONSORSHIP Applications may be requested beginning the 3rd week of March. Program deadline is August 15, as long as funding is still available. Eligible: CTGR Tribal members grades K-12. , - . TUTORIAL SERVICES Program provides tutorial services at Willamina Middle and High School. In-class individual, after-school availability. Eligible: CTGR Tribal members and JOM eligible students (must be enrolled in a Federally Recognized Tribe). NATIVE AMERICAN CLUBS Clubs meet bi-weekly at Wil lamina Middle and High School. Opportunity to participate in cultural, leadership, and educa tion activities, field trips, and conferences. Eligible: CTGR Tribal members and JOM eligible students (must be enrolled in a Federally Recog nized Tribe).
