APPOINTMENTS : (503)370-200 1-800-152-7823 We try to schedule appointments so that patients have a minium of waiting; however, we will see walk-in patients on a work-in basis as the schedule allows. Physical exams, well-child care, and first prenatal visits should be scheduled at least a week in advanced to allow mple time to serve you. When you call, please use your full name rather than nicknames. This helps to quickly locate your medical record for your appointment. If it is your first visit to the clinic, please try to get there about twenty (20) minutes before your appointment to fill out an application for services. When you arrive, please check in with the receptionist and sign in. Let her Know that you are there to see tne doctor, or to utilize one of the other srevices at the clinic. Please be prepared for the possibility of a short wait. We try to get people on time, but serving an individual's health needs is not always predictable. EMERGENCIES: During regular hours, emergency cases wi i 1 be seen immediately by the clinic staff; after regular clinic hours those within the service area please go to the Salem Memorial Hospital emergency room. Ambulance service can be obtained by dialing "911". In an emergency, a patient eligible for contract care may go directly to a health care facility without prior authorization by IMS. Authorization must be sought by the patient's family or physician within 72 hours. I will take your name and address by phone if necessary. I have alot of requests to fill so please be patient with me in sending the forms to you. Even if you're already on the final membership roll of 1956, you still must re-enroll with the tribe, as it is very important that we have current information on all of our tribal members. The Bureau of Indian Affairs need current addresses and also they will be re questing an iupdated list of all members listed on the membership roll of 1956 that are deceased. Please let me remind all of you' who have not gotten a copy of your original birth certificate to me for your files to do so in the very near future because the Bureau is going to ask that the family histories and also those copies be submitted to them for review in the near future. Baptismal copies are going to be accepted by the Bureau in place of birth certificate copies. I hope you all had a Merry Christmas and a Happy New Year in 1 98-4. Cheryl Carl Tribal Enrollment Clerk
