Prevention Team survey
for a better community
The Confederated
Tribes of Warm Springs
is conducting an anony-
mous survey of Tribal
members.
This survey will help
the tribal Prevention
Team collect informa-
tion, so alcohol and drug preven-
tion services and programs can be
adapted to meet the members’
needs.
Your responses are voluntary
and private. Once your
data is collected, there
is the option to enter
your name into a raffle
drawing (different link
than the one containing
your answers). Five
items with an estimated
value of $30 per item will be
awarded at the end of the survey
collection period. Thank you for
your input! Warm Springs Pre-
vention Team.
