This short term program gives veterans who are in need of housing , the opportunity to address unemployment, addiction recovery, basic living skills and homclcssncss which may have prevented them from living as independent life. Goals: The Department of Veterans Affairs Urban Domiciliary is a participatory and multi-disciplinary program whose primary goal is rehabilitation. The Domiciliary will focus on improving health, renewing of self-carre and social skills, upgrading academic and vocational abilities, and assisting in community placement. The intention of the Dimiciliary program is to prepare the veteran to function at hisher optimal level of independence. Objectives: Provide temporarytransitional living space for veterans who need minimal physiological, sociological, and psychological services. Veterans referred from an inpatient psychiatric facility must have pre-arranged outpatient treatment and continue in that treatment while in the Domiciliary. Referring services of agencies must submit a discharge summary prior to the veteran's admission. If applying from outside the VA Medical Center please mail discharge summaries to VA Medical Center (ll-DOM), P.O. Box 1035, Portland, Oregon 97207. Veterans need to be able to care for themselves, maintain their own medications, do their own laundry, and keep appointments. Veterans must be able to perforin routine activities of daily living. Veterans must exhibit the following characteristics to gain admission: Motivation to participate in the Domiciliary programs. Willingness to adhere to Domiciliary rules and regulations. Ability to CO-exist in a semi structured community setting. Willingness to maintain an alcoholdrug free environment. Application for admission to the Domiciliary program will be made through the Domiciliary Social Worker or his assistant. Final review of application will take place in the Domiciliary Admission Screening Committee. Veterans seeking admission to the program who are chemically addicted and who have been practicing their addiction at any time within the last 12 months must successfully complete an inpatient addiction treatment program prior to entry into the Domiciliary. Successful completion is determined by the Domiciliary Admission Screening Committee .Veterans seeking treatment for their addiction should coniact the Portland Medical Center Chemical Addiction Rehabilitation Service (CARS) in Vancouver, Washington. This reckless incident could have put the lives of police officers & residents in the area, in danger. Greg Jackson, Jr. Howlak Tichum Louis "Jimmy" LeClaire, III Louis LeClaire III, age 4 1 years, died as a result of an automobile accident in Lapwai, Idaho on Au gust 3, 1997. LeClaire was born April 24. 1956 to Louis LeClaire Jr. and Naomi Janice VanPelt in Redmond, Oregon. He lived in Warm Springs with his grandmother Delia LeClaire until her passing in 1988 then resided in Lapwai, Idaho. He had attended school in Warm Springs and Madras. LeClaire was working and living in Lapwai, Idaho at his passing. Preceded in death are grand mother Delia LeClaire who died March 23, 1988, brothers Watkins LeClaire who died in 1979andBrian LeClaire who died in 1962. i Survivors include his parents, brothers Thomas, Louis Dean, Marlon, Wallace, David and Ezekial LeClaire; sisters Donna Janet, , Regina Leslie, Denise Gail, Lorna Kit and Cynthia Darcy Michele ; among numerous relatives and friends. Dressing ceremony was held in Idaho with a memorial service fol lowing at Lapwai. He was trans ported to the Warm Springs Shaker Church on Monday, August 4, where overnight services were held and buried the following day on Tuesday, August 5 at the Tenino Cemetery. J 1
