Request for Proposals
Qualified contractors are requested to subm it proposals for
furnishing worksite modification assistance to em ployers and
injured workers who are clients of the W orkers C om pensa­
tion Division. Services required include job analysis, ergo­
nomic assessment, worksite m odification equipm ent and
vendor research, coordination of worksite modification ser­
vices, and worksite modification follow-up. Such services are
necessary in areas throughout the state.
Proposals will be accepted by the W orkers Com pensation
Division, Operations Section until 5:00 p.m. Aug. 1 5 ,1 994. A
copy of the Request for Proposal (RFP) and the terms and
conditions of the contract may be obtained from Becky
Sherwood, Operations Section, W orkers' Compensation Di­
vision, 2nd Floor, Labor and Industries Bldg., Salem OR,
97310, or by calling (503) 945-7617.
Any or all proposals may be rejected for good cause upon a
finding that it is in the public interest to do so.
